摘  要:目的探讨益肾活血止痛方治疗骨转移癌的可能作用机制。方法 40只SD大鼠随机分为中药组、唑来膦酸组、模型组、假手术组,每组10只。除假手术组外其余各组采用大鼠乳腺癌Walker 256细胞悬液3μl缓慢注射于大鼠胫骨骨髓腔内建立骨转移癌模型。于造模后第5天开始给药,唑来膦酸组给予唑来膦酸溶液于第5、7、9、12、14、16、19天腹腔注射,每次30μg/kg;中药组给予益肾活血止痛方0.5 ml(浓度1.92 g/ml)灌胃,假手术组及模型组给予等量生理盐水0.5 ml灌胃,每日1次,连续3周。观察各组大鼠肿瘤体积,X线摄片对骨质破坏程度进行评分,HE染色观察胫骨组织病理形态;检测胫骨组织中骨保护素(OPG)、核因子κB受体活化因子(RANK)、核因子κB受体活化因子配体(RANKL)蛋白及mRNA表达。结果与模型组比较,中药组和唑来膦酸组大鼠胫骨肿瘤体积及骨质破坏减少,胫骨组织OPG蛋白及mRNA表达升高,RANKL、RANK蛋白及mRNA表达降低(P<0.05)。中药组和唑来膦酸组各指标组间比较差异无统计学意义(P>0.05)。结论益肾活血止痛方可抑制大鼠骨转移癌的生长,其机制可能与其激活胫骨组织OPG表达,抑制RANKL和RANK的活化有关。Objective To explore the possible mechanism of Yishen Huoxue Zhitong Decoction( 益肾活血止痛方)in the treatment of bone metastases. Methods A total of 40 SD rats were randomly divided into Chinese medicine group,zoledronic acid group,model group and sham operation group,with 10 rats in each group. Except the sham operation group,3 μl of rat breast cancer Walker 256 cell suspension was slowly injected into the bone marrow cavity of rat tibia to establish a bone metastasis model in other groups. The drug was administered on the 5 th day after modeling,and the zoledronic acid group was given zoledronic acid solution intraperitoneally on the 5 th,7 th,9 th,12 th,14 th,16 th,and 19 th day,with 30 μg/kg each time; Yishen Huoxue Zhitong Decoction 0. 5 ml( concentration 1. 92 g/ml) was intragastrically administered. The sham operation group and the model group were given normal saline 0. 5 ml once a day for 3 weeks. The tumor volume of each group was observed. The degree of bone destruction was scored by X-ray film. The pathological morphology of the tibia was observed by HE staining. The osteoprotegerin( OPG) and nuclear factor κB receptor activating factor( RANK),nuclear factor kappa B receptor activating factor ligand( RANKL) protein and mRNA expressio were detected in the tibia. Results Compared with the model group,the sacral tumor volume and bone destruction were decreased in the Chinese medicine group and the zoledronic acid group,and the expression of OPG protein and mRNA in the tibia was increased,The expression of RANKL,RANK protein and mRNA were decreased( P〈0. 05). There was no significant difference between the Chinese medicine group and the zoledronic acid group in all indicators( P〉0. 05). Conclusion Yishen Huoxue Zhitong Decoction can inhibit the growth of rat bone metastases,and its mechanism may be related to the activation of OPG expression in tibia and the inhibition of RANKL and RANK activation.
