摘  要:椎间盘退变是引起腰背部疼痛的主要原因,遗传、年龄、营养状态、长期负重所致的组织弱化是造成椎间盘退变的基本因素。目前对于椎间盘退行性疾病的治疗可分为三类:保守治疗、手术治疗、修复和再生策略。保守和手术治疗只能缓解腰背部疼痛症状,不能从根本上治疗引起退变的原因。同时,手术治疗不能逆转椎间盘退变,甚至会进一步加重现有的损伤。椎间盘的修复和再生策略通过重塑椎间盘正常的生理结构和功能来治疗椎间盘退行性疾病,并且不会增加椎间盘损伤,其主要方式有细胞治疗、组织工程学等。目前针对椎间盘再生的研究主要集中在髓核再生。尽管纤维环修复和再生是髓核及椎间盘修复的关键,这方面的研究却相对较少。近来,椎间盘再生策略已经越来越注重同时修复受损的纤维环和髓核,以防止或推迟单独修复髓核所致的退行性改变。本文将讨论椎间盘修复和再生的治疗策略和成就,并提出未来的研究方向。The aim of regeneration and repair, including cell therapy and tissue engineering(TE), for degenerated intervertebral disc is restoring its normal structure and function without additional injury. Currently the researches on this field mainly focus on nucleus pulposus regeneration, whereas less relate to the annulus fibrosus repair or regeneration although the annulus fibrosus is the key point in reversing degenerative progression of intervertebral disc. However, the recent strategy of regeneration and repair is increasingly revolving around mending the annulus fibrosus and nucleus pulposus together to prevent further degeneration after repairing nucleus pulposus only. The purposes of this paper are to review the therapeutic strategies and achievements in intervertebral disc repair and regeneration, and explore the prospect of research in the future.
