摘  要:目的探讨罗红霉素抑制慢性阻塞性肺疾病(COPD)大鼠炎性介质释放对糖皮质激素抵抗的影响。方法将30只裸大鼠按随机数字表法分为模型组(n=10)、罗红霉素组(n=10)和空白组(n=10),模型组与罗红霉素组均建立COPD模型,空白组不予以建模;建模完成后,模型组以生理盐水灌胃,罗红霉素组以罗红霉素灌胃,空白组正常饲养,不予处理。持续30 d后,检测大鼠腹主动脉血白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、单核细胞趋化蛋白-1(MCP-1)水平及肺组织糖皮质激素受体α(GRα)及GRβ表达水平。结果与空白组比较,模型组血清IL-8、TNF-α、MCP-1水平明显增高(P<0.05),肺组织GRαmRNA及蛋白表达明显降低(P<0.05),GRβmRNA及蛋白表达明显升高(P<0.05)。与模型组比较,罗红霉素组血清IL-8、TNF-α、MCP-1水平明显降低(P<0.05),肺组织GRαmRNA及蛋白表达明显升高(P<0.05),GRβmRNA及蛋白表达明显降低(P<0.05)。结论罗红霉素通过抑制炎性介质释放,能够调节GRα/GRβ平衡,从而有助于改善COPD大鼠糖皮质激素抵抗状态。Objective To investigate the effect of roxithromycin inhibiting the release of inflammatory mediators on glucocorticoid resistance in rats with chronic obstructive pulmonary disease( COPD). Methods Thirty nude rats were randomly divided into model group( n = 10), roxithromycin group( n = 10) and blank group( n = 10). The COPD model was established in model group and roxithromycin group. After modeling, the model group was treated with intragastric administration of normal saline while roxithromycin group was treated with intragastric administration of roxithromycin. The blank group was fed normally,without any treatment. Thirty days later, the levels of interleukin-8( IL-8), tumor necrosis factor alpha( TNF-α),monocyte chemoattractant protein-1( MCP-1),g lucocorticoid receptor alpha( GRα) and GRβ in the abdominal aortic blood of rats were detected. Results The levels of IL-8,TNF-α and MCP-1 in model group were significantly higher than those in blank group( P〈0. 05). The expression of GRα mRNA and protein in lung tissues of model group was significantly lower than that in blank group( P〈0. 05),while the expression of GRβ mRNA and protein was significantly higher than that in blank group( P〈0. 05). Serum levels of IL-8,TNF-α and MCP-1 in roxithromycin group were significantly lower than those in model group( P〈0. 05). The expression of GR mRNA and protein in lung tissues of roxithromycin group was significantly higher than that in model group( P〈0. 05),while the expression of GRβ mRNA and protein was significantly lower than that in model group( P〈0. 05). Conclusion Roxithromycin can regulate the balance of GRα/GRβ by inhibiting the release of inflammatory mediators,which contributes to the improvement of glucocorticoid resistance in COPD rats.
