经皮椎体成形术与保守治疗胸腰椎压缩性骨折的疗效比较

摘　　要：目的回顾性分析比较经皮椎体成形术与保守治疗骨质疏松引起的胸腰椎压缩性骨折的疗效。方法回顾性分析2010年1月至2015年1月在该院治疗的160例骨质疏松引起的胸腰椎压缩性骨折患者,其中84例行PVP治疗(PVP组),76例接受保守治疗(保守治疗组),比较2组患者临床疗效和影像学指标。结果 PVP组术后1周及1个月视觉模拟评分与保守治疗组比较,差异均有统计学意义(P<0.05);但术后6、12个月比较,差异均无统计学意义(P>0.05)。PVP组术后1周Oswestry功能障碍指数评分与保守治疗组比较,差异有统计学意义(P<0.05)。术后1、6、12个月比较,差异均无统计学意义(P>0.05)。2组术前、术后1周及术后1、6、12个月Cobb角比较,差异均无统计学意义(P>0.05)。PVP组术前伤椎椎体前缘高度百分比与保守治疗组比较,差异无统计学意义(P>0.05);术后1周及1、6、12个月比较,差异均有统计学意义(P<0.05)。结论经皮椎体成形术为早期治疗骨质疏松引起的压缩性骨折的首选治疗方法,能明显缓解患者疼痛。Objective To retrospectively compare the efficacy of percutaneous vertebroplasty（PVP）with conservative treatment for patients with thoracolumbar osteoporotic compression fractures. Methods A total of 160 patients with thora-columbar osteoporotic compression fractures in the hospital from January 2010 to January 2015 were retrospectively analyzed.Among 84 patients got PVP treatment（PVP group）and 76 patients got conservative treatment（conservative treatment group）.The clinical effect and radiological changes in both groups were compared. Results After treatment in 1 week and 1 month,the difference between visual analogue scale（VAS）of the PVP group and that of the conservative treatment group was statisti-cally significanton（P0.05）;after treatment in 6,12 months,the differences were not statistically significant（P0.05）.Aftertreatment in 1 week,the difference between Oswestry disability index score of the PVP group and that of the conservative treat-ment group was statistically significanton（P0.05）;after treatment in 1,6,12 months,the differences were not statistically sig-nificant（P0.05）.At before the treatment and after treatment in 1 week,and 1,6,12 months,the difference between the Cobbangle of the PVP group and those of conservative treatment group were not statistically significanton（P0.05）.At before thetreatment,the vertebral ratio of 2 groups had no statistical significance（P0.05）;after treatment in 1 week,and 1,6,12 months,the differences were statistically significant（P0.05）. Conclusion PVP is the first choice of early treatment for thora-columbar osteoporotic compression fractures,can significantly relieve pain of patients.
