摘  要:目的观察唐山地区汉族醛固酮合成酶基因CYP11B2-344T/C多态性与冠心病的相关性。方法采用病例对照研究方法,选取2012年3月—2014年2月华北理工大学附属医院心内科连续收治的诊断为冠心病及胸痛待查患者,采用定量冠状动脉造影(QCA)方法观察冠状动脉狭窄程度,任一支冠脉血管狭窄≥50%者为冠心病组,任一支冠脉血管狭窄<50%或无明显狭窄者为对照组,记录临床资料,多聚酶链反应-限制性片段长度多态性分析(PCR-PFLP)检测醛固酮合成酶CYP11B2-344T/C位点基因型。结果(1)冠心病组与对照组比较,TT、TC、CC三种基因型频率分别为45.2%、45.2%、9.6%vs.54.0%、36.7%、9.3%,TC+CC基因型频率分别为54.8%vs.46.0%,T、C等位基因频率分别为67.8%vs.72.3%、32.7%vs.27.7%,差异均无统计学意义(χ~2=1.129,P=0.209;χ~2=2.907,P=0.099;χ~2=3.382,P=0.037)。(2)冠脉多支病变组与单支病变组比较,TT、TC、CC三种基因型频分别为42.4%、46.1%、11.5%vs.47.8%、44.8%、7.5%,TC+CC基因型频率分别为57.6%vs.52.2%。T、C等位基因频率分别为65.5%vs.70.1%、34.5%vs.29.9%,差异均无统计学意义(χ~2=1.082,P=0.582;χ~2=0.551,P=0.469;χ~2=0.947,P=0.384)。结论唐山汉族CYP11B2-344T/C基因多态性与冠心病未见明显相关性。Objective To study the relationship between CYP11 B2-344 T/C genetic polymorphisms and susceptibility to Coronary Heart Disease in Tangshan.Methods A case-control study was carried out.From March 2012 to Februry 2014,400 inpatients were admitted in author's hospital.The coronary lesions were evaluated by quantitative coronary angiography(QCA).The coronary heart disease group referred to coronary artery stenosis greater than50%,while the control group was defined as less than50%coronary angiographic stenosis,or no significant stenosis.The gender,age,history of smoking,drinking,hypertension,diabetes mellitus,family situation and drug use were recorded.Aldosterone Synthase gene CYP11 B2-344 T/C gene polymorphism were detected by polymerase chain reaction-restriction fragment length polymorphism(PCR-PFLP).Results In coronary group and control groups,the genotype frequencies of TT,TC,CC were45.2%(113/250),45.2%(113/250),9.6%(24/250)vs 54.0%(81/150),36.7%(55/150),9.3%(14/150).The frequency of TC+CC genotypes was 54.8%(137/250)and 46.0%(69/150)respectively.The frequency of T,C allele was 67.8% vs 72.3%;32.7% vs 27.7%.There were no statistically differences(χ^2=1.129,P=0.209;χ^2=2.907,P=0.099;χ^2=3.382,P=0.037).Between multi-vessel group and single coronary artery group,the genotype frequencies of TT,TC,CC were 42.4%(70/165),46.1%(76/165),11.5%(19/165)vs47.8%(32/67),44.8%(30/67),7.5%(5/67).The frequency of TC+CC genotypes was 57.6%(95/165)vs52.2%(35/67).The frequency of T,C allele was 65.5% vs 70.1%,34.5% vs 29.9%.The differences were not statistically significant(χ^2=1.082,P=0.582;χ^2=0.551,P=0.469;χ^2=0.947,P=0.384).ConclusionThere are no relationships between CYP11 B2-344 T/C gene polymorphism and coronary disease in Tangshan Han population
