摘  要:目的研究健康儿童冠状动脉内径随体表面积(body surface area,BSA)变化的关系,构建最佳BSA校正的冠状动脉内径Z值(Z scores)计算公式。方法选择2013年3月—2017年4月在河北省人民医院就诊的509例心脏结构正常儿童,年龄1d^15.2岁,测量身高、体质量,计算BSA,采用彩色多普勒超声心动图仪器测量右冠状动脉主干(RCA)、左冠状动脉主干(LMCA)、左前降支(LAD)、左回旋支(LCX)的内径。检验自变量BSA与因变量冠状动脉内径大小的Pearson相关系数,建立二者线性、指数、对数、幂、平方根回归模型,检验模型的异方差性以及Z值是否服从正态分布,确定最佳模型以计算Z值。结果冠状动脉内径与年龄、身高、体质量、BSA、(BSA)^(1/2)显著相关(Pearson相关系数r值范围0.674~0.817,P<0.05),其中与(BSA)^(1/2)的相关性优于BSA(P<0.05)。以BSA为自变量构建的平方根模型Z值呈正态分布,随机误差项方差相等,调整决定系数(R^2a)高于指数模型(P<0.05),适合于低年龄组儿童,可以其为最佳模型计算不同BSA的儿童冠脉内径Z值。结论平方根模型是计算儿童冠脉内径Z值较为准确的方法。Objective To study the relationship between the coronary artery diameter and the body surface area (BSA) in healthy children,and establish the best BSA corrected formula of Z value ( scores) of coronary artery diameter.Methods From March 2013 to April 2017,509 patients were eligible for inclusion with normal heart structure that was diagnosed in author's hospital,with the age of 1 day to 15.2 years.The height and body mass of the children were measured and their BSA was calculated.Color Doppler echocardiography was used to measure the inner diameter of proximal right coronary artery (RCA) ,left main coronary artery ( LM- CA), left anterior descending coronary artery (LAD), and left circumflex artery ( LCX ).The Pearson correlation coefficient between the independent variable BSA and the size of the internal diameter of the dependent variable was checked,and the linear, exponential, logarithmic,power and square root regression models were established,respectively.And then we tested the heteroscedastieity of the regression model and whether the Z value obeys the normal distribution to determine the best model to calculate Z value.Results The diameter of coronary artery was significantly correlated with age, height, body weight,BSA and (Pearson correlation coefficient r value was 0.674- 0.817,P〈0.05),and the correlation between was superior to BSA.The Z value of the square root model was normal distribution, and the variance of the random error was equal.The adjusted R -square (R2a) was higher than the exponential model (P〈0.05), which was suitable for children in the small age group.The optimal model was established and the Z value of coronary artery diameter in healthy children with different BSA could be calculated by the best model.Conclusion The square root model is a more accurate model for calculating the Z value of coronary artery diameter in children.
