摘　　要：目的探究射频导管消融(RFCA)治疗儿童快速型心律失常(TA)的疗效、安全性、复发率及对不同年龄段患儿治疗效果的影响。方法收集湖州市中心医院和浙江大学医学院附属第一医院在2014年1月至2017年12月收治的100例行RFCA治疗的TA患儿为研究对象,比较不同年龄段的消融效果、不同类型TA的消融效果、复发因素及并发症的发生情况。结果在100例患儿中,消融成功97例,成功率为97.00%;复发8例,复发率为8.00%。不同类型TA患儿年龄和消融成功率比较均无显著性差异(F=0.25,P=0.78;χ~2=2.35,P=0.09),各组复发率比较却存在显著性差异(χ~2=4.35,P=0.04),但经年龄分层后,5~8岁组、9~14岁组的RFCA成功率、复发率、并发症发生率比较均无显著性差异(χ~2值分别为0.10、0.25、0.15,均P>0.05)。结论 RFCA可作为治疗TA患儿有效、安全的手术方法,其手术成功率高,复发率低,且不同年龄组的患儿手术效果、复发率和安全性相当。Objective To study the efficacy,safety and recurrence rate of radiofrequency catheter ablation（RFCA）in the treatment of children with tachycardiac arrhythmia（TA）and the effect of age on treatment outcomes.Methods One hundred children with TA treated by RFCA in Huzhou Central Hospital and The First Affiliated Hospital of Zhejiang University were selected as research objects from January 2014 to December 2017.The ablation effect of different age groups,the effect on TA by different types of ablation,the factors of recurrence and the incidence of complications were compared.Results Of the 100 cases,97 cases were successfully ablated,with the success rate of 97.00%,and 8 cases relapsed,with the recurrence rate of 8.00%.There was no significant difference in age and ablation success rate among children with different types of TA（F=0.25,P=0.78,χ^2=2.35,P=0.09）,but there was significant difference in recurrence rate among different groups（χ^2=4.35,P =0.04）.There was no significant difference in RFCA success rate,recurrence rate and the incidence rate of complications between 5-8 years old group and 9-14 years old group（χ^2 value was 0.10,0.25 and 0.15,respectively,all P〉0.05）.Conclusion RFCA can be used as an effective and safe operation for children with TA with high success rate and low recurrence rate.The surgical effect,recurrence rate and safety of children in different age groups are comparable.
