信阳市医疗机构法定传染病漏报调查分析

摘　　要：目的了解信阳市各级各类医疗机构法定传染病报告管理工作的现状,为进一步提高传染病网络直报质量提供依据。方法在全市范围内抽取45家医疗单位进行传染病报告情况的调查。结果信阳市各医疗机构法定传染病漏报率为9.70%,县及县以上综合医院的漏报率低于乡村医疗单位(χ2=14.295,P<0.01)。使用和未使用电子病历系统的法定传染病漏报率分别为7.35%和2.65%,差异无统计学意义(χ2=1.245,P=0.154)。门诊和住院部的漏报率分别为13.19%和4.27%,门诊漏报现象严重(χ2=6.460,P<0.05)。乙、丙类法定传染病漏报率分别为9.16%和9.70%,差异无统计学意义(χ2=0.077,P=0.781)。本次调查的法定传染病报告及时率为99.47%;传染病卡片填写的完整率、准确率和一致率分别为93.58%、85.56%和88.77%。其中专科医院传染病报告的准确率和一致率均低于县及县以上综合医院和乡村医疗单位(χ2=16.001,P<0.01;χ2=20.857,P<0.01),分别为70.37%和72.22%。结论信阳市医疗机构法定传染病报告质量偏低,主要表现在法定传染病漏报率高,报告的准确性、一致性差。各医疗卫生机构应积极采用措施和手段进一步提高报告质量。Objective The present study was conducted to assess the management of notifiable infectious diseases in medical institutions at all levels in Xinyang city and provide evidence for the further improvement of the reporting quality. Methods According to stratified random sampling requirements,45 medical institutions were investigated in Xinyang City. Results The average missing report rate of notifiable infectious diseases was9. 70% in medical institutions in Xinyang City. The missing report rates of comprehensive medical institutions at county and above county level were lower than that of rural medical institutions.（ χ^2= 14. 295,P〈0. 01）. The missing report rates of notifiable infectious diseases by using and un-using electronic medical record system were 7. 35% and 2. 65% respectively. The difference was not statistically significant（ χ^2= 1. 245,P = 0. 154）.The missing report rates in departments of outpatient and inpatient were 13. 19% and 4. 27%,the missing reports of outpatient department was worse（ χ^2= 6. 460,P〈0. 05）. The missing report rates of class B and C notifiable infectious diseases were 9. 16% and 9. 70%respectively,with no significant difference（ χ^2= 0. 077,P = 0. 781）. The rate of timely reporting was 99. 47%.The rates of integrity, accuracy and consistency were93. 58%,85. 56% and 88. 77%. The rates of accuracy and consistency of specialized medical institutions were70. 37% and 72. 22%,the lowest among various medical institutions（ χ^2= 16. 001,P〈0. 01; χ^2= 20. 857,P〈0. 01）. Conclusion The quality of notifiable infectious diseases reporting in medical institutions was low in Xinyang city. The average missing report rate of notifiable infectious diseases was high. The accuracy and consistency of the report were poor. It is still necessary to conduct various measures and methods for the further improvement.
