摘 要:目的探讨黄芩甙预处理对心肌缺血/再灌注损伤(MI/RI)大鼠血流动力学及肌钙蛋白I浓度的影响。方法选取健康SD大鼠60只,采用数字表法将大鼠随机分为三组:假手术组、模型组、黄芩甙组,各20只。从造模前2周开始,对黄芩甙组大鼠腹腔注射黄芩甙,同时对另两组给予同剂量生理盐水;给药两周后,对模型组、黄芩甙组结扎左冠状动脉前降支(LAD)复制MI/RI模型,假手术组只用丝线穿过LAD、但不结扎;在心肌缺血/再灌注前、缺血30 min、再灌注60 min及再灌注120 min时,检测各组血流动力学参数、心肌活性氧(ROS)水平及血浆肌钙蛋白I浓度。结果 (1)黄芩甙组ROS ligated水平、肌钙蛋白I浓度分别为(116.33±5.21)、(4.34±1.98)ng/m L,与模型组的(138.67±7.68)、(6.33±2.36)ng/m L比较明显降低,差异均有统计学意义(P<0.05);(2)与模型组比较,黄芩甙组大鼠SOD、GSH-Px活性明显升高、MDA含量明显降低,差异均有统计学意义(P<0.05);(3)再灌注120 min时,黄芩甙组心率、左室收缩末压力、左室舒张末压力、左室内压最大上升速率、左室内压最大下降速率分别为(332.2±32.7)次/min、(92.1±9.8)mm Hg、(-2.3±1.6)mm Hg、(431±129)mm Hg/s、(481±132)mm Hg/s,与模型组的(285.6±26.4)次/min、(69.4±9.7)mm Hg、(3.6±2.1)mm Hg、(251±121)mm Hg/s、(267±173)mm Hg/s比较,各参数之间差异均有统计学意义(P<0.05);(4)再灌注120 min时,黄芩甙组心率、左室收缩末压力、左室内压最大上升、下降速率回升,而左室舒张末压力回降,接近缺血30 min时,差异均无统计学意义(P>0.05)。结论黄芩甙预适应,可启动、形成和储备更强的内源性抗氧化损伤的能力,能抵抗更长时间心肌缺血/再灌注导致的损伤,从而改善MI/RI大鼠血流动力学,保护心肌结构和心脏功能。Objective To discuss the influences of baicalin pretreatment on hemodynamics and cTnI concentration in rats with myocardial ischemia/reperfusion injury(MI/RI). Methods Sixty healthy SD rats were selected and divided into three groups according to random number table: sham operation group, model group, baicalin group, each with 20 rats. Two weeks before model preparation, rats in baicalin group were given peritoneal injection of baicalin, and those in other two groups were given the same amount of saline. After drug administration for two weeks, left anterior descending coronary artery(LAD) was ligated to copy MI/RI model in model group, baicalin group, while the LAD was only threaded with silk thread in sham operation group. The hemodynamic parameters, and myocardial reactive oxygen species(ROS) level, and plasma cTnI concentration of the three groups were detected before MI/RI, at 30 min of ischemia, 60 min of reperfusion and 120 min of reperfusion. Results(1) The ROS level, cTnI concentration of baicalin group were(116.33±5.21),(4.34±1.98) ng/m L respectively, as compared with(138.67±7.68),(6.33±2.36) ng/m L in model group(all P〈0.05).(2) Compared with model group, the SOD, GSH-Px activity of the rats in baicalin group significantly increased, and the content of MDA significantly decreased(all P〈0.05).(3) At 120 min of reperfusion, heart rate, left ventricular end systolic pressure, left ventricular end diastolic pressure, maximum increase rate of left ventricular pressure,maximum drop rate in the left ventricle pressure of baicalin group were(332.2±32.7) times/min,(92.1±9.8) mm Hg,(-2.3 ± 1.6) mm Hg,(431 ± 129) mm Hg/s,(481 ± 132) mm Hg/s, respectively, as compared with(285.6 ± 26.4) times/min,(69.4±9.7) mm Hg,(3.6±2.1) mm Hg,(251±121) mm Hg/s,(267±173) mm Hg/s in model group(all P〈0.05).(4)At 120 min of reperfusion, heart rate, left ventricular end systolic pressure, maximum rate of rise and f
