摘 要:目的探讨生物型股骨假体髋关节置换术对老年股骨颈关节置换术患者骨代谢指标及关节功能的影响。方法选择2015年7月~2016年10月收治的老年股骨颈骨折患者112例为研究对象,采用前瞻性研究方法随机分为观察组和对照组各56例,对照组给予混合型假体髋关节置换术,观察组给予生物型股骨假体关节置换术,比较两组手术相关指标、骨代谢指标、Harris髋关节评分、并发症等指标。结果观察组手术时间明显短于对照组,住院时间明显长于对照组(t=3.142,8.112,P<0.05,P<0.01);血清BALP含量明显高于对照组,DPD、TRACP5b含量明显低于对照组(t=5.174,3.802,9.184,P<0.05,P<0.01);肺部感染等并发症(8.93%)明显低于对照组(25.00%)(χ~2=5.134,P<0.05)。结论生物型股骨假体髋关节置换术有助于提高老年股骨颈骨折患者的骨代谢能力,促进髋关节功能恢复,减少术后并发症的发生。Objective To investigate the effect of hip replacement by femoral bioprosthesis on bone metabolism indices and joint function in elderly patients with femoral neck joint replacement. Methods A total of 112 elderly patients with femoral neck fractures who were admitted from July 2015 to October 2016 were selected as the study subjects. Using prospective research methods, they were randomly divided into observation group and control group, with 56 cases in each group. The control group was given mixed prosthesis hip replacement. The observation group was given hip re- placement by femoral bioprosthesis. The relevant indicators, bone metabolism indicators, Harris hip score, complica- tions and other indicators were compared between the two groups. Results The surgery time was significantly shorter in the observation group than in the control group. The length of hospital stay was significantly longer than the control group(t=3.142, 8.112, P〈0.05, P〈0.01); the serum BALP content was significantly higher than that in the control group. The contents of DPD and TRACPSb were significantly lower than those in the control group (t=5.174, 3.802, 9.184, P〈 0.05, P〈0.01); complications such as pulmonary infection of 8.93% were significantly lower than that of 25.00% in the control group(x2=5.134, P〈0.05). Conclusion Hip replacement by femoral bioprosthesis is beneficial to improving bone metabolism in elderly patients with femoral neck fractures, promoting hip function recovery, and reducing postoperative complications.
