氯沙坦和缬沙坦对中青年高血压合并高尿酸血症及血清补体的影响

摘　　要：目的探讨氯沙坦和缬沙坦对中青年高血压合并高尿酸血症及血清补体的影响。方法将2016年6月~2017年9月我院收治的128例中青年高血压合并高尿酸血症患者分为氯沙坦治疗的氯沙坦组,缬沙坦治疗的缬沙坦组;治疗前、4周及结束时检测血压、血尿酸、C3及C4补体水平,并记录不良反应发生情况。结果治疗4周时及治疗结束时氯沙坦组和缬沙坦组收缩压及舒张压均显著降低(P<0.05);治疗4周时及治疗结束时氯沙坦组和缬沙坦组血尿酸均明显降低;治疗4周时及治疗结束后氯沙坦组及缬沙坦组患者血中C3补体及C4补体均显著降低(P<0.04);治疗4周时及治疗结束时氯沙坦组患者血中C3补体及C4补体水平显著低于缬沙坦组;氯沙坦组1例出现头晕反应,缬沙坦组未出现不良反应。结论对中青年高血压合并高尿酸血症患者采用氯沙坦和缬沙坦治疗均具有较理想的降血压作用,但相较于缬沙坦,氯沙坦可有效降低血尿酸及补体水平。Objective To investigate the differences of the efficacy of losartan and valsartan in the treatment of young and middle-aged patients with hypertension complicated with hyperuricemia and their effects on serum complement. Methods A total of 128 young and middle-aged patients with hypertension complicated with hyperuricemia who were admitted to our hospital from June 2016 to September 2017 were divided into losartan group （losartan treatment） and valsartan group （valsartan treatment）. Blood pressure, serum uric acid, C3 and C4 complement levels were measured be- fore treatment, 4 weeks after treatment and at the end of treatment, and the occurrence of adverse reactions was recorded. Results At week 4 after treatment and at the end of treatment, the systolic and diastolic blood pressures in losartan group and valsartan group were significantly decreased （P〈0.05）; serum uric acid in losartan group and valsar- tan group was significantly decreased at week 4 after treatment and at the end of treatment; at week 4 after treatment and at the end of treatment, the levels of serum C3 complement and C4 complement in the losartan group and valsartan group were significantly decreased（P〈0.05）; at week 4 after treatment and at the end of treatment, the levels of C3 com- plement and C4 complement in the blood of patients in losartan group were significantly lower than those in valsartan group; one case in losartan group had dizziness, and there was no adverse reaction in valsartan group. Conclusion For young and middle-aged patients with hypertension complicated with hyperuricemia have a better antihypertensive effect when they are treated with losartan and valsartan. However, losartan can effectively reduce serum uric acid and comple- ment levels compared with valsartan.
