Monaco计划系统和Pinnacle计划系统在鼻咽癌容积旋转调强放射治疗剂量验证的对比研究

摘　　要：目的探讨Monaco计划系统和Pinnacle计划系统在鼻咽癌容积旋转调强放射治疗(VMAT)中基于Arc CHECK模体进行剂量验证,并据此分析不同的参数下的剂量对比研究。方法选择40例行根治性放射治疗的鼻咽癌患者,病理学分期为Ⅰa~Ⅱb期(UICC/AJCC2002分期)。依Monaco计划系统和Pinnacle计划系统各20例。设计计划用ArcCHECK模体进行验证。采用Gamma分析法在对比标准2%/3 mm、3%/3 mm、3%/5 mm条件下,比较两种计划系统验证通过率;采用DTA分析法,在对比标准3%/3 mm,比较两种计划系统绝对剂量和相对剂量的通过率并进行非参数检验分析。结果 Monaco计划系统的通过率高于Pinnacle计划系统;在Gamma分析法、标准2%/3 mm和3%/3 mm时,两种计划系统通过率差异有统计学意义(P<0.05);而在对比标准3%/5 mm时,通过率差异无统计学意义(P>0.05)。采用DTA距离符合度分析法下,两种计划系统的相对剂量和绝对剂量均有统计学意义(P<0.05)。结论 Monaco计划系统和Pinnacle计划系统均能验证完成鼻咽癌VMAT的计划,Monaco计划系统较Pinnacle有着更高的验证通过率。Abstact： Objective To explore clinical planning accuracy of Pinnacle and Monaco treatment planning systems（TPS） by com-paring dose verification results of volume modulated arc therapy（VMAT） in treatment of nasopharyngeal carcinoma（NPC）, and study the feasibility in VMAT dosimetric verification based on the Arc CHECK module. Methods Atotal of 40 cases of NPC patients undergoing radical radiation therapy were enrolled, the pathological stage was stage Ⅰ-Ⅱ b（UICC/AJCC2002 stage）and divided into 2 system by Monaco planning system（n = 20） and Pinnacle planning system（n = 20）. The design plan was validated with Arc CHECK phantom. The threshold（TH） was 10 %, Gamma analysis were used at standard 2 %/3 mm, 3 %/3 mm and 3 %/5 mm to compare verification rates between 2 systems. Deference to agree（DTA） analysis were performed at standard conditions of 3 %/3 mm, TH = 10 % to compare passing rate of relative dose distribution and absolute dose distribution. Results The passing rate of Monaco TPS was higher than that of Pinnacle TPS. There were significant differences between Monaco TPS and Pinnacle TPS on Gamma passing rate at standard 2 %/3 mm and 3 %/3 mm（ P 0.05）, while there were no significant differences at standard 3 %/5 mm（P 〉0.05）. By using DTA method, the relative and absolute doses of 2 TPS were significantly different（P 〈0.05）. Conclusion It is demonstrated that Monaco and Pinnacle TPS could be applied to clinical VMAT, and accuracy of Monaco TPS is higher than that of Pinnacle TPS.
