摘 要:目的 探讨甲亢患者甲状腺激素水平与心肌酶学改变的相关性。 方法 选取2017年1月至2017年12月该院内科门诊及住院部临床确诊为单纯甲亢的100例患者作为研究对象,同期在体检中心查体的健康者100例为对照组。所有研究对象均晨起抽取静脉血检测血清TT3、TT4、FT3、FT4 以及促甲状腺激素(TSH)、天门冬酸氨基转移酶(AST)、肌酸激酶(CK)、CK 的同工酶(CK-MB)、乳酸脱氢酶(LDH)、LDH 同工酶(α-HBDH)含量。比较两组的甲状腺激素水平和心肌酶谱水平;对两组患者的心肌酶谱与甲状腺激素水平进行Pearson相关分析。 结果 研究组TT3、TT4、FT3、FT4和TSH均高于对照组( P <0.05),差异具有统计学意义;研究组AST、CK、CK-MB、LDH及α-HBDH均高于对照组( P <0.05),差异具有统计学意义;研究组的AST、CK、CK-MB、LDH及α-HBDH均与TT3、TT4、FT3、FT4呈正相关(均0< r <1, P <0.05),与TSH呈负相关(均-1< r <0, P <0.05);对照组的AST、CK、CK-MB、LDH及α-HBDH均与TT3、TT4、FT3、FT4及TSH无相关性(均 r =0)。 结论 检测甲亢患者的心肌酶谱,有助于了解其物质代谢与能量代谢的紊乱情况,对于有可能导致的心脏损伤具有一定预测价值。Objective: To investigate the correlation between thyroid hormone levels and myocardial enzymes in patients with hyperthyroidism. Methods 100 patients with hyperthyroidism were enrolled in this study from January 2001 to December 2017,and 100 healthy subjects selected as the control group at the same time in the physical examination center.The levels of TT3,TT4,FT3,FT4 and thyroid stimulating hormone (TSH),aspartate aminotransferase (AST),creatine kinase (CK),CK isoenzyme (CK-MB),lactate dehydrogenase (LDH) and LDH isoenzyme (α-HBDH) were measured by intravenous blood samples from all subjects.The levels of thyroid hormones and myocardium were compared between the two groups.Pearson correlation analysis was performed between the two groups with myocardial enzymes and thyroid hormone levels. Results : The levels of TT3,TT4,FT3,FT4 and TSH in the study group were higher than those in the control group ( P 〈0.05),the difference was statistically significant.The levels of AST,CK,CK-MB,LDH and α-HBDH in the study group were higher than those in the control group ( P 〈0.05),the difference was statistically significant.The AST,CK,CK-MB,LDH and α-HBDH of the study group were positively correlated with TT3,TT4,FT3,FT4 (all 0 〈 r 〈1, P 〈0.05),negatively correlated with TSH (all- ( P 〈0.05).The levels of AST,CK,CK-MB,LDH and α-HBDH in the control group were not correlated with TT3,TT4,FT3,FT4 and TSH ( r =0). Conclusion: The detection of myocardial enzymes in patients with hyperthyroidism can help to understand the disorder of metabolic and energy metabolism in patients with hyperthyroidism,which may be of predictive value for possible cardiac damage.
