血清生长分化因子-15在原发性高血压初诊患者中的变化

摘　　要：目的观察原发性高血压初诊患者血清生长因子(GDF)-15的变化及相应临床意义。方法选取87例初诊的原发性高血压患者为观察组,另选取同期87例体检的正常成人为对照组,对比分析两组研究对象的临床资料,观察两组血清GDF-15水平及舒张期室间隔厚度(IVST)、甘油三酯(TG)、左心室质量指数(LVMI)、左室后壁厚度(PWT)、空腹血糖(FBG)、糖化血红蛋白(Hb A1c)、超敏C反应蛋白(hs-CRP)、高密度脂蛋白胆固醇(HDL-C)、左房内径(LAD)、低密度脂蛋白胆固醇(LDL-C)、左室舒张末期内径(LVDd)、总胆固醇(TC)及其相关性。结果观察组GDF-15、TG、PWT、IVST、LVMI优于对照组,HDL-C水平低于对照组,差异有统计学意义(P<0.05);其他指标水平两组比较差异无统计学意义(P>0.05)。GDF-15与TG、PWT、IVST、LVMI均呈正相关(r=0.182、0.207、0.311、0.281,P<0.05),与HDL-C呈负相关(r=-1.234,P<0.05)。结论原发性高血压初诊患者的血清GDF-15水平有所升高,与相关脂质代谢及左室重构指标存在一定的相关性。Objective To observe the changes of serum growth factor-15 in patients with newly diagnosed essential hypertension and their corresponding clinical significance.Methods 87 patients with newly diagnosed essential hypertension were selected as observation group,another 87 healthy adults in the same period were selected as control group,the clinical data of research objects in the two groups were compared and analyzed.The serum GDF-15 level and diastolic interventricular septum thickness（IVST）,triglyceride（TG）,left ventricular mass index（LVMI）,left ventricular posterior wall thickness（PWT）,fasting blood glucose（FBG）,glycosylated hemoglobin（Hb A1 c）,hypersensitive C reactive protein（hs-CRP）,high density lipoprotein cholesterol（HDL-C）,and left atrial diameter（LAD）,low density lipoprotein cholesterol（LDL-C）,left ventricular end diastolic diameter（LVDd）,total cholesterol（TC） and their correlation between the two groups were compared.Results The GDF-15,TG,PWT,IVST,LVMI of the observation group were significantly better than those of the control group,the HDL-C level was significantly lower than the control group,the differences were statistically significant（P〈0. 05）.There were no statistically significant differences in other indicators between the two groups（P〈0. 05）.GDF-15 was positively correlated with TG,PWT,IVST and LVMI（r=0.182,0.207,0.311,0.281,P〈0. 05）,and negatively correlated with HDL-C（r=-1.234,P〈0. 05）.Conclusion Serum level of GDF-15 is elevated in newly diagnosed patients with essential hypertension,which is correlated with lipid metabolism and left ventricular remodeling.
