清痹愈风汤治疗湿热痹阻型强直性脊柱炎疗效观察

摘　　要：目的:观察清痹愈风汤联合西药治疗湿热痹阻型强直性脊柱炎的临床疗效。方法:将湿热痹阻型强直性脊柱炎患者116例随机分为观察组与对照组各58例。对照组予甲氨蝶呤肌肉注射、柳氮磺吡啶肠溶片口服治疗,观察组在治疗组治疗的基础上加用自拟方清痹愈风汤治疗,2组疗程均为3个月,比较治疗前后强直性脊柱炎活动指数(BASDAI)、功能指数(BASFI)及中医证候积分,同时检测治疗前后红细胞沉降率(ESR)、C反应蛋白(CRP)和免疫球蛋白A(Ig A)等。结果:1)观察组与对照组总有效率分别为93.1%和79.3%,2组比较差异有统计学意义(P<0.05)。2)2组治疗后BASDAI、BASFI指数和中医证候积分均减少,与治疗前比较差异有统计学意义(P<0.05),观察组减少较对照组更明显(P<0.05)。3)治疗前后2组CRP、ESR及Ig A水平均下降(P<0.05),观察组较对照组下降更明显(P<0.05)。结论:自拟清痹愈风汤联合西药可显著缓解湿热痹阻型强直性脊柱炎患者的临床症状,改善脊柱关节功能。Objective： To observe clinical effects of QingBi YuFeng decoction combined with western medicine in treating ankylosing spondylitis （AS） of damp-heat arthralgia pattern. Methods： All 116 AS patients were randomized into the observation group and the control group, 58 cases each group. The patients in the control group accepted intramuscular injection of methotrexate and oral administration of Sulfasalazine enteric-coated tablets, the patients in the observatioin group took selfmade prescription QingBi YuFeng decoction on the foundation of the therapy the observation group accepted, the course was three months for both groups, TCM syndrome scales, Bath Ankylosing Spondylitis Disease Activity Index （BASDAI） and Bath Ankylosing Spondylitis Functional Index （BASFI） were compared before and after treating, meanwhile, immunoglobulin A （IgA）, C reactive protein （CRP） and erythrocyte sedimentation rate （ESR） were detected before and after treating. Results： 1） Total effective rates of the observation group and the control group were 93.1% and 79.3% respectively, and the difference had statistical meaning （P〈0.05）. 2） BASDAI, BASFI and TCM syndrome scales of both groups decreased after treating, and the difference showed statistical meaning when they were compared with before treating （P〈0.05）, the decrease of the observation group was more obvious than the that of the control group （P〈0.05）. 3） The levels of CRP, ESR and IgA of both groups lowered significantly （P〈0.05）, the decrease of the observation group was more obvious than that of the control group （P 〈0.05）. Conclusion： Selfmade QingBi YuFeng decoction combined with western medicine could notably alleviate clinical symptoms of AS patients of damp-heat arthralgia pattern, and improve the functions of articulationes colunmae.
