摘 要:膨胀纹具有较高的发生率,临床上分为红色膨胀纹和白色膨胀纹,不同类型膨胀纹的色素、血管、胶原等方面的变化存在差异。根据这些靶色基的变化特点,可以选择相应的光电治疗手段进行针对性治疗。本文通过文献分析发现:以血红蛋白为主要靶基的激光及强脉冲光对红色膨胀纹治疗具有较好的疗效,不良反应小。有增加黑色素效应的激光对白色膨胀纹有效。促进胶原新生、重排的点阵CO_2激光及非剥脱点阵激光能够有效的改善白色和红色膨胀纹,其中非剥脱点阵激光不良反应较小。多种形式联合治疗或者加用具有生长因子等细胞因子可以提高疗效。但是目前的文献,都存在样本量偏小,诊断标准及研究方案不统一等缺陷,这使得各种技术之间无法进行直接比较。Striae distensae( SD) represents a common type of permanent dermal lesions,which is classified as striae rubra( SR) and striae alba( SA) by appearance. Those lesions differed in melanin,vascular architecture and collagen,which means that individualized treatment modality should be selected according to the changes of these target chromophores. The use of vascular lasers can improve SR by targeting hemoglobin,whereas increasing melanin through ultraviolet light or excimer laser is effective for in treating SA. Increasing collagen production by fractional photoelectric technologies,such as fractional CO2,non ablative fractional laser and plasma is popular for SD. Non ablative fractional laser is optimum in terms of therapeutic results and side effect. The combined therapy of multiple energy based modalities or addition of growth factors will improve the therapeutic effect. The previous studies,however,are limited due to the small sample size and indefinite diagnostic criteria and study protocol,making it difficult to compare these available modalities directly.
