摘  要:目的探讨多层螺旋CT(MSCT)三维重建对气管支气管异物的诊断价值。方法随机收集2013年1月至2017年12月支气管异物患儿22例,均经支气管镜诊治,男9例,女13例,年龄1~9岁,发病距CT检查时长1 h^2.5个月。回顾性分析其肺MSCT三维重建表现,评估其显示情况及诊断价值。结果 22例患儿CT诊断异物部位与内镜结果一致。右侧16例,左侧6例。CT异物直接征象:支气管腔内异物,较小异物附着于腔内壁,且伴气道狭窄2例;较大异物嵌塞于气道内19例,导阻塞气道;未见直接征象1例。CT异物间接征象:肺气肿15例,伴纵隔双边影7例;肺不张2例;肺炎3例;无间接征象2例。结论 MSCT三维重建可明确诊断支气管异物,可作为支气管异物术前定位定性诊断手段,对指导纤支镜异物取出术具有重要意义。Objective To investigate the diagnostic value of multi-slice spiral CT (MSCT) three-dimensional (3D) reconstruction in diagnosis of tracheobronchial foreign bodies. Methods Twen- ty-two chilren with bronchial foreign body from January 2013 to December 2017 were randomly collected by bronchoscopy, including 9 males and 13 females, aged at 1 to 9 years old, and the time of the inci- dence to CT examination was 1 h to 2. 5 months. The MSCT 3 D reconstruction of lung was analyzed retro- spectively, and its indication and diagnostic value were evaluated. Results The CT diagnosis of foreign bodies in 22 children was consistent with endoscopic findings. There were 16 cases on the right side and 6 cases on the left side. The direct signs of CT foreign bodies showed that: the foreign bodies in the tra- cheal cavity, the smaller foreign bodies attached to the inner wall of the cavity and airway stenosis in 2 cases, the larger foreign bodies were impacted in the airway, resulting in obstruction of the airway in 19 cases, and no direct signs were found in 1 case. Indirect signs of CT foreign bodies: emphysema was found in 15 cases, bilateral mediastinal shadow was found in 7 cases, atelectasis was found in 2 cases, pneumonia was found in 3 cases, and absence was found in 2 cases. Conclusions The three-dimensional reconstruction of MSCT can clearly diagnose the tracheobronchial foreign bodies bronchus, and can be used as a qualitative diagnostic method for the preoperative localization of bronchial foreign body, which is of great significance for guiding the removal of the foreign body of the bronchoscope.
