激素联合手术治疗浆细胞性乳腺炎合并乳头内陷的效果观察

摘　　要：目的:探讨分析激素联合手术治疗浆细胞性乳腺炎合并乳头内陷的效果。方法:选取2011年3月-2017年2月本院乳腺外科收治的110例浆细胞性乳腺炎合并乳头内陷患者作为研究对象,按照随机数字表法将其分为观察组和对照组,各55例。观察组给予激素联合手术治疗,对照组仅给予手术治疗,比较两组患者的临床疗效、切口愈合情况、治疗前后QOL评分和VAS评分及术后复发情况。结果:治疗后观察组患者总有效率为96.36%,明显高于对照组的83.64%,差异有统计学意义(X^2=9.804,P=0.007)。观察组患者切口愈合情况优于对照组,差异有统计学意义(X^2=4.681,P=0.031)。治疗前两组患者QOL、VAS评分比较,差异均无统计学意义(P>0.05);治疗后两组患者QOL评分均明显高于治疗前,VAS评分均明显低于治疗前,且观察组均优于对照组,差异均有统计学意义(P<0.05)。观察组患者术后总复发率为5.45%,明显低于对照组的18.18%,差异有统计学意义(X^2=4.274,P=0.039)。结论:激素联合手术治疗浆细胞性乳腺炎合并乳头内陷可提高临床效果,降低患者疼痛,改善生活质量,降低术后复发率,保留患者的乳房外形,建议在临床上推广。Objective：To explore and analyse the effect of hormone combined with surgery in the treatment of plasma cell mastitis with nipple retraction.Method：A total of 110 patients with plasma cell mastitis complicated with nipple retraction from March 2011 to February 2017 in Galactophore Department of our hospital were selected.According to the random number table method,they were divided into observation group and control group,55 cases in each group.The observation group was treated with hormone combined with surgery treatment,the control group was only treated with surgery treatment.The clinical efficacy,incision healing,QOL and VAS scores before and after treatment and postoperative recurrence between the two groups were compared.Result：The total effective rate of the observation group was 96.36%,which was significantly higher than 83.64% of the control group,the difference was statistically significant（χ^2=9.804,P=0.007）.The incision healing in the observation group was better than that in the control group,the difference was statistically significant（χ^2=4.681,P=0.031）.There were no significant differences in QOL and VAS scores between the two groups before treatment（P〈0.05）;after treatment,the QOL scores of the two groups were significantly higher than those before treatment,the VAS scores were significantly lower than those before treatment,and the observation group were better than those in the the control group,the differences were statistically significant（P〈0.05）.The postoperative total recurrence rate of the observation group was 5.45%,which was significantly lower than 18.18% of the observation group,the difference was statistically significant（χ^2=4.274,P=0.039）.Conclusion：Hormone combined with surgery in the treatment of plasma cell mastitis with nipple retraction can improve clinical efficacy,reduce patient＇s pain,improve the quality of life,reduce postoperative recurrence rate,preserve breast shape of patients,it is recommended that it be promoted clinically.
