VGCC抗体在带状疱疹中的临床意义研究

摘　　要：目的:探讨血清电压门控钙通道(voltage-gated calcium channel,VGCC)在带状疱疹中的意义,为临床更好的治疗带状疱疹(herper zoste,HZ)以及预防带状疱疹后神经痛(post herpetic neuralgia,PHN)的发生提供理论依据。方法:收集带状疱疹患者50例,在入院、出院当天采用视觉模拟评分法(visual analoguescale,VAS)进行疼痛评分及ELISA法检测血清中VGCC抗体的浓度,于出院后1月随访是否发生PHN。检测20例健康志愿者血清VGCC抗体的浓度为对照。结果:50例带状疱疹患者中9例发生了PHN,发生率18%。带状疱疹患者治疗前血清VGCC抗体浓度与治疗后及健康志愿者相比,显著降低(P<0.05);发生PHN与未发生PHN的VGCC抗体浓度比较,显著降低(P<0.05);不同疼痛程度组血清VGCC抗体浓度比较差异均有统计学意义(P<0.05)。结论:急性期VGCC抗体含量的检测以及疼痛程度的评定对预测PHN的发生可能具有一定价值。Objective：To investigate the significance of serum voltage-gated calcium channel（VGCC）in herpes zoster,and to provide a theoretical basis for clinical treatment of herpes zoster and prevention of post-herpetic neuralgia（PHN）.Methods：A total of 50 patients with herpes zoster were enrolled;the visual analogue scale（VAS）was used to determine pain score,and ELISA was used to measure the serum level of VGCC antibody.The patients were followed up at 1 month after discharge to observe the presence or absence of PHN.A total of 20 healthy volunteers were enrolled as control group,and the serum level of VGCC antibody was also measured.Results：Of all 50patients with herpes zoster,9 developed PHN,resulting in an incidence rate of 18%.The patients with herpes zoster had a significant reduction in the serum level of VGCC antibody after treatment and a significantly lower level than healthy volunteers（P〈0.05）.The patients who developed PHN had a significantly lower than those who did not in the serum level of vacc antibody（P〈0.05）.There was a significant difference in the serum level of VGCC antibody between the groups with different pain scores（P〈0.05）.Conclusion：The measurement of VGCC antibody and the evaluation of pain degree in acute stage may help to predict the development of PHN.
