超声引导下收肌管阻滞对膝关节镜清理术患者术后镇痛效果的影响

摘　　要：目的:探讨超声引导下收肌管阻滞(U-ACB)对膝关节镜清理术(KAM)患者术后镇痛效果的影响。方法:选取膝关节镜清理术患者100例,随机分为收肌组和对照组,每组50例,对照组给予静脉镇痛治疗,收肌组在此基础上给予U-ACB治疗。结果:收肌组术后12、24、48h的VAS得分、镇痛泵按压次数、不良反应发生率明显低于对照组,收肌组术后12、24、48h膝关节活动度明显高于对照组(P<0.05),收肌组和对照组术后12、24、48h股四头肌肌力基本相同(P>0.05)。结论:U-ACB可有效提高KAM患者术后镇痛效果,有利于膝关节早期活动和减少不良反应,其对股四头肌肌力无明显不良影响。Objective： To discuss the effect of adductor canal block under ultrasound guidance（U-ACB） on analgesia effect in patients after knee arthroscopy myotubes（KAM）. Methods： 100 patients after KAM were selected from June 2014 to June 2017, according to the random distribution, all patients were divided into adductor group and control group, 50 cases in each group, control group was given intravenous analgesia treatment, adductor group on the basis was given U-ACB. Results： The after 12, 24, 48 h VAS score, analgesia pressing times, adverse reactions rate of adductor group were significantly lower than those of control group, the after 12, 24, 48h knee joint activity of adductor group were significantly higher than those of control group（P〈0.05）, the after 12, 24, 48h muscle strength of four biceps femoris of adductor group and control group were basically the same（P〉0.05）. Conclusion： U-ACB can improve the analgesic effect in patients with KAM. It is conducive to the early knee activity and reduce adverse reaction. It has no adverse effect on the muscle strength of femoral head four.
