摘　　要：目的探讨糖尿病患者血清中s CD40L、TNF-α与患者视网膜病变程度、肾脏病变程度及心血管事件发生的关系。方法选取本院2015年6月-2016年6月收治的确诊为2型糖尿病的患者80例作为研究对象,所有患者评价视网膜病变、肾脏病变严重程度,采用酶联免疫吸附法检测患者血清中s CD40L、TNF-α水平,并随访患者心血管事件的发生。结果非糖尿病视网膜病变组患者s CD40L、TNF-α水平明显低于糖尿病视网膜病变组患者(P<0.05)。非糖尿病肾病组患者s CD40L、TNF-α水平明显低于糖尿病肾病组患者(P<0.05),上述差异均有统计学意义。多因素分析结果提示s CD40L、TNF-α是糖尿病患者心血管事件的独立危险因素(P<0.05)。结论糖尿病患者血清中s CD40L、TNF-α水平与患者视网膜病变程度、肾功能程度密切相关,是患者心血管事件发生的独立危险因素。Objective To investigate the relationship between the levels of serum sCD40L, TNF-α and microvascular lesions and cardiovascular events in patients with diabetes mellitus. Methods Eighty patients diagnosed as type 2 diabetes were enrolled as the research objects in our hospital from June 2015 to June 2016, and all patients were conducted for the detection of microvascular lesions and renal lesions. Serum sCD40L and TNF-α levels were measured by enzyme-linked immunosorbent assay（ELISA）. The occurrence of cardiovascular events was followed up. Results The levels of sCD4OL and TNF-α in patients without diabetic retinopathy were significantly lower than those in patients with diabetic retinopathy （P〈 0.05）. The levels of sCD40L and TNF-α in the non-diabetic nephropathy group were significantly lower than those in the diabetic nephropathy group, with the differences statistically significant （P〈0.05）. Multivariate analysis suggested that sCD40L and TNF-α were independent risk factors for cardiovascular events in diabetic patients （P〈0.05）. Conclusion The levels of serum sCD40L and TNF-α in diabetic patients are closely related to the degree of retinopathy and renal function, which are independent risk factors for the occurrence of cardiovascular events in oatients.
