摘  要:目的:观察针刺联合前列地尔对噪音性耳聋大鼠耳蜗毛细胞血管内皮生长因子(vascular endothelial growth factor,VEGF)、Bax及Bcl_2表达的影响。方法:32只雄性Wistar大鼠按照随机数字法分为4组:空白对照组、模型组、前列地尔治疗组和针刺联合前列地尔治疗组。除对照组外,其余3组制备噪音性耳聋大鼠模型。对各组大鼠进行听觉脑干诱发电位(auditory brainstem response,ABR)听力阈值测试。联合治疗组的针刺治疗穴位选取三焦经腧穴翳风(双)、外关(双)。对各组大鼠进行耳蜗VEGF、Bax及Bcl-2表达检查。结果:造模结束后,各组大鼠ABR听力阈值显著高于对照组;模型组VEGF水平较空白对照组低,耳蜗Bcl-2 mRNA表达减弱,Bax mRNA表达增强,差异有统计学意义(P<0.05);联合治疗组与模型组相比,大鼠耳蜗基底膜毛细胞VEGF水平显著恢复,耳蜗Bcl-2 mRNA表达增强,Bax mRNA表达减弱,差异有统计学意义(P<0.01)。结论:针刺联合前列地尔治疗能改善噪声导致的听力损害,其改善听力、保护耳蜗结构是通过调节耳蜗基底膜毛细胞VEGF、耳蜗相关凋亡因子Bcl-2、BaxmRNA的表达而达到的。Objective:To observe the effect of acupuncture combined with alprostadil on the expression of vascular endothelial growth factor( VEGF),Bax and Bcl-2 in the cochlear capillaries of noise induced deafness rats. Methods:32 male Wistar rats were randomly divided into the blank control group,the deafness model group,the prostadil treatment group and the acupuncture combined with prostadil treatment group. The noise induced deafness rat model was prepared except for control group. Rats in each group accepted auditory brainstem response( ABR) testing. Acupuncture treatment of acupuncture combined treatment group selected meridian acupoints Yifeng( double),Waiguan( double). The expression of VEGF,Bax and Bcl-2 in the cochlear hair cells was examined at the end of the treatment. Results:The ABR hearing threshold of rats in each group was significantly higher than that in the control group. Compared to the noise deafness model group and blank control group,the level of rat cochlea hair cells of VEGF decreased significantly,decreased expression of Bcl-2 in cochlea of mRNA,enhance the expression of Bax mRNA,there were statisticallysignificant difference(P〈0. 05). Combined treatment group compared with the model group,the rat cochlea hair cell immune VEGF level recovered significantly,enhanced expression of cochlear Bcl-2 mRNA,Bax mRNA expression decreased,there were statistically significant difference(P〈0. 01). Conclusion:Acupuncture combined with alprostadil can improve hearing impairment caused by noise. The improvement of hearing and cochlear structure is achieved by regulating the expression of VEGF,cochlear related apoptosis factor Bcl-2 and BaxmRNA in the cochlear basement membrane.
