声带白斑保守治疗效果及影响因素分析

摘　　要：目的:探讨声带白斑保守治疗选择及影响因素。方法:回顾性分析给予保守治疗并定期随访的84例声带白斑患者的临床资料,对保守治疗无效或病情进展的病例及时采取手术治疗。结果:获得所有患者的完整随访资料,保守治疗有效组初诊年龄,保守治疗时间分别为(51.50±12.92)岁,(184.63±86.98)d,无效组分别为(53.46±12.29)岁及(191.54±84.88)d,差异无统计学意义(P>0.05)。保守治疗有效组病程为(360.68±574.97)d,无效组为(1213.46±1017.87)d,病程长短分析差异具有统计学意义(P<0.05)。两组从发病原因、饮酒史、合并反流症状比较,差异存在统计学意义(P<0.05)。结论:声带白斑保守治疗效果与患者有无明确病因发病,发病时间长短,以及是否合并反流症状有一定的相关性。Objective：To discuss the choice of conservative treatment of vocal cord leukoplakia and its influence factors.Methods：Clinical data of 84 patients with vocal cord leukoplakia were analyzed retrospectively.All patients were given conservative treatment and regular follow-up,for cases with no treatment effect or suffering disease progression,surgical treatments were given in time.Results：All patients received complete follow-up data.The age at the beginning of the conservative treatment effective group was（51.50±12.92）years old（184.63±86.98）days,while the invalid group was（53.46±12.29）years old and（191.54±84.88）days）The difference was not statistically significant（P 〉0.05）.The course of the conservative treatment was（360.68±574.97）days in the effective group,and（1213.46±1017.87）days in the invalid group.The difference in duration of disease was statistically significant（P 〈 0.05）.There were statistically significant differences between the two groups in terms of the cause of the disease,the history of drinking,and the symptoms of combined reflux（P 〈0.05）.Conclusion：The effect of conservative treatment of vocal cord leukoplakia has certain correlation with patient’s specific causes,duration of the disease and whether there was a combined reflux event.
