高血压知识对老年高血压患者服药依从性影响研究

摘　　要：目的了解60岁及以上老年高血压患者服药的依从性现状及其对高血压知识了解程度,探讨高血压知识的掌握程度对老年高血压患者服药依从性的影响,为制定有效的干预措施提供理论依据。方法运用多阶段随机抽样方法选取武汉市4个区543名60岁及以上的老年高血压患者,以一对一问卷调查老年高血压患者的服药依从率和高血压相关知识了解现状,并分析高血压知识对服药依从性的影响。结果老年高血压患者的服药依从率为67.0%;老年高血压患者的高血压相关知识总平均得分为16.96±3.39分,其中农村为15.91±3.48分,显著低于城区的18.38±2.67分(P<0.05)。单因素和多因素分析结果显示,药物治疗方面知识的掌握度对老年高血压患者服药依从性有影响(P<0.05)。结论老年高血压患者的高血压知识掌握程度影响患者服药依从性,建议加强高血压知识的宣传,尤其是药物治疗相关知识的健康教育。Objective To understand the status of medication adherence and the level of knowledge about hypertension among hypertensive patients aged over 60 years old in Wuhan city and analyze the effect of knowledge of hypertension on medication compliance in hypertensive patients,and to provide evidences for improving medication adherence.Methods Mult-stage random sampling method was used to select 543 elderly hypertensive patients aged 60 and over from four districts in Wuhan.Questionnaire interviews were conducted by person to person to analyze the influence of high blood pressure knowledge on medication compliance.Results The prevalent rate of medication adherence among elderly hypertensive patients in Wuhan city was 67.0%.The general average score of hypertension related knowledge of elderly hypertensive patients was 16.96±3.39,of which,the countryside was 15.91±3.48,and the urban area was 18.38 ± 2.67.The results of the univariate and multivariate analysis showed the main factors influencing the medication adherence were the knowledge of medical treatment（P0.05）.Conclusion The knowledge on hypertension in elderly hypertensive patients can affect the medication compliance of elderly patients with hypertension.It is suggested to strengthen the knowledge of hypertension,especially the health education of medical treatment to improve the compliance of medication in senile hypertensive patients.
