摘  要:目的探讨狂犬病疫苗"2-1-1"免疫程序联合人狂犬病免疫球蛋白在狂犬病预防中的作用。方法回顾性分析我中心收治的230例因动物咬伤引起的暴露后Ⅱ、Ⅲ级损伤患者。其中对照组(110例)接受传统狂犬疫苗"5针法"免疫程序联合人狂犬病免疫球蛋白治疗,观察组(120例)接受狂犬病疫苗"2-1-1"免疫程序联合人狂犬病免疫球蛋白治疗。比较两组患者抗狂犬病抗体阳性率及不良事件发生情况。结果观察组患者接种7d抗狂犬病抗体阳性情况优于对照组(P<0.05),接种14d两组患者抗狂犬病抗体均转为阳性,两组比较无差异(P>0.05)。对照组Ⅱ、Ⅲ级暴露患者抗狂犬病抗体阳性率分别为96.00%、94.29%,观察组分别为97.50%、90.00%,两组Ⅱ、Ⅲ级暴露患者抗狂犬病抗体阳性率比较无差异(P>0.05)。两组患者全身反应及局部反应不良事件发生率比较无差异(P>0.05)。结论"2-1-1"免疫程序与人狂犬病免疫球蛋白联合预防狂犬病效果较佳,可在短时内提高抗狂犬病抗体数量,阻止病毒扩散,值得推荐使用。Objective To investigate the effect of rabies vaccine "2-1-1"immunization program combined with human rabies immunoglobulin on rabies prevention. Methods A retrospective analysis of 230 patients with grade II and III injuries caused by animal bites in our hospital from April 2016 to December 2016 was carried out. The control group received the traditional "5 acupuncture"immune program treatment and the experimental group received rabies vaccine"2-1-1"immune program combined with human rabies immunoglobulin treatment. The anti rabies antibody positive situation and adverse events were compared between the two groups. Results The positive rate of anti rabies antibody in the experimental group was better than that in the control group 7 days after the treatment( P 0. 05). The anti rabies antibodies turned positive in both groups 14 days after the treatment,and there was no difference between the two groups( P 0. 05). The positive rates of anti-rabies antibodies in patients with grade Ⅱ and Ⅲ exposure in the control group were 96. 00% and94. 29%,respectively. The positive rates of anti-rabies antibodies in patients with grade Ⅱ and Ⅲ exposure in the experimental group were 97. 50% and 90. 00%,respectively. There was no difference in anti-rabies antibody positive between the two groups of patients with grade Ⅱ and Ⅲ exposure( P 0. 05). There was no significant difference in the incidence of systemic reactions and local adverse events between the two groups( P 0. 05). Conclusion The effect of "2-1-1"immune program and human rabies immune globulin combined with rabies prevention is better. It can improve the number of anti rabies antibodies in a short time and prevent the spread of the virus which is recommended to use.
