摘 要:目的探讨血尿酸水平与2型糖尿病患者心-踝血管指数(CAVI)的相关性。方法纳入2型糖尿病但尚未进展至糖尿病肾病患者135例,记录患者一般临床资料,血生化指标、CAVI。根据CAVI四分位数将患者分为四组,分析CAVI与血尿酸的关系及相关影响因素与CAVI的相关性。结果 (1)CAVI中位数9.45,根据CAVI四分位数将患者分为四组,分别为:Q1≤8.2(n=36);Q2:8.2~9.45,(n=34);Q3:9.45~10.45,(n=33);Q4≥10.45,(n=32)。四组间年龄、体质指数(BMI)、低密度脂蛋白胆固醇(LDL-C)、估算肾小球滤过率(e GFR)均存在统计学差异(P<0.05);(2)四组患者血尿酸(SUA)水平存在统计学差异(P<0.05),SUA Q4组显著高于1、2、3组(P<0.05);血尿酸与糖尿病患者CAVI呈正相关(r=0.365,P<0.01);(3)CAVI水平与年龄呈正相关,与e GFR呈负相关(r=-0.215,P<0.05)。结论血尿酸对2型糖尿病患者动脉硬化有一定影响。血肌酐正常的糖尿病患者早期e GFR的降低可能是发生动脉硬化的危险因素之一。Objective To investigate the correlation between serum uric acid level and heart-ankle vascular index(CAVI) in type 2 diabetic patients. Methods 135 patients with type 2 diabetes and without diabetic nephropathy were enrolled. We recorded the general clinical data, blood biochemical indicators, and CAVI. The patients were divided into four groups according to CAVI quartile. The relationship between CAVI and serum uric acid and the correlation between CAVI and CAVI were analyzed. Result(1)CAVI median 9.45, according to CAVI quartile patients were divided into four groups, respectively: Q1≤8.2, 36 cases; Q2: 8.2-9.45, 34 cases; Q3: 9.45-10.45, 33 cases; Q4≥10.45, 32 cases. There were significant differences in age, body mass index(BMI), low density lipoprotein cholesterol(LDL-C), estimated glomerular filtration rate(e GFR) between the four groups(P〈0.05).(2)SUA level was higher than that of Q1, Q2 and Q3 groups(P〈0.05). There was a positive correlation between serum uric acid and CAVI in patients with diabetes mellitus(r=0.365, P〈0.01).(3)The level of CAVI was positively correlated with age and negatively correlated with e GFR(r=-0.215, P〈0.05). Conclusion Serum uric acid has a certain effect on arteriosclerosis in type 2 diabetes mellitus patients. Decreased early e GFR in diabetic patients with normal serum creatinine may be one of the risk factors of atherosclerosis.
