盐酸替扎尼定联合针灸对脑卒中上肢痉挛患者上肢运动功能及生活质量的影响

摘　　要：目的探讨盐酸替扎尼定联合针灸对脑卒中上肢痉挛患者上肢运动功能及生活质量的影响。方法选取医院2015年4月至2017年6月收治的120例脑卒中上肢痉挛患者作为观察组,给予盐酸替扎尼定联合针灸治疗;另按随机平衡原则选取2013年1月至2015年2月收治的120例脑卒中上肢痉挛患者作为对照组,给予A型肉毒毒素联合针灸治疗。两组疗程均为1个月。结果治疗后,两组患者肌张力等级[改良Ashworth量表(MAS)评分]均有显著改善(U_观=4.871,U_对=3.412,P<0.05),且观察组改善幅度优于对照组(U=3.814,P<0.05);Fugl-Meyer量表(FMA)评分、改良Barthel指数(MBI)评分、生活质量量表(SF-36)评分均显著上升,且观察组明显高于对照组(P<0.05);治疗期间,观察组仅出现1例疲乏、1例嗜睡,不良反应发生率为1.67%。结论盐酸替扎尼定联合针灸治疗脑卒中上肢痉挛对患者肌张力、日常生活活动能力的改善较A型肉毒毒素联合针灸治疗更显著,可使患者获得更佳的运动功能及生活质量,且安全性良好。Objective To investigate the effect of tizanidine hydrochloride combined with acupuncture and moxibustion on the motor function of upper limbs and quality of life in patients with stroke and upper limb spasticity.Methods Totally 120 patients with stroke and upper limb spasticity admitted to our hospital from April 2015 to June 2017 were selected as the observation group which was given tizhnitine hydrochloride combined with acupuncture and moxibustion.According to the principle of random equilibrium,another 120 patients with stroke and upper limb spasticity admitted to our hospital from January 2013 to February 2015 were selected as the control group,which was given intramuscular injection of Botulinum Toxin Type A for Injection combined with acupuncture and moxibustion.The two groups were treated for 1 month.Results After treatment,the levels of muscle tension of the two groups [ modified Ashworth scale（MAS）] were significantly improved（U（observation）= 4.871,U（control） = 3.412,P〈0.05）,and the improvement in the observation group was more significant than that in the control group（U = 3.814,P〈0.05）.The scores of Fugl-Meyer assessment（FMA）,modified Barthel index（MBI） and quality of life scale（SF-36） in the two groups were significantly increased,and the scores of the observation group were significantly higher than those of the control group（P〈0.05）.There was only 1 case of fatigue and 1 case of lethargy in the observation group during treatment,and the incidence rate was 1.67%.Conclusion Compared with Botulinum Toxin Type A combined with acupuncture and moxibustion,tizanidine hydrochloride combined with acupuncture and moxibustion can more obviously improve the muscle tension,ADL,motor function and quality of life of patients with stroke and upper limb spasticity,and it has good safety.
