摘  要:目的:探讨四维彩超联合胎儿颈项部皮肤透明厚度(nuchal translucency,NT)超声检测诊断胎儿畸形价值。方法:收集孕11~14周的孕妇311例进行胎儿NT检查,孕22~28周进行四维彩超检查。结果:NT增厚者20例,其中2例行引产处理;18例做染色体检查的孕妇中4例发现染色体异常,均做引产处理;4例孕妇分别在孕14周、18周、21周增厚的NT逐渐消退,1例自然流产,9例NT增厚但染色体正常孕妇四维彩超正确检出率为88.89%;311例孕妇中发生胎儿畸形24例,发生率为7.72%(24/311),而NT超声联合四维彩超的正确检出率为91.67%(22/24)。结论:孕早期行胎儿NT超声可降低胎儿畸形风险,孕中期行四维彩超进一步明确目标胎儿的发育状况,避免胎儿畸形漏检。Objective: To explore the value of four-dimensional color Doppler ultrasound combined with fetal NT ultrasound in the diagnosis of fetal malformation. Methods: 311 pregnant women were collected and those who had been pregnant for 11 to 14 weeks were examined by fetal NT ultrasound,and those who had been pregnant for 22 to 28 weeks were examined by four-dimensional color Doppler ultrasound. Results: There were 20 cases of NT thickening,2 cases of which were treated with induction of labor; the thickening of NT of 4 pregnant women gradually disappeared after 14 weeks,18 weeks,and 21 weeks; there was 1 case of spontaneous abortion; there were 9 cases of NT thickening with normal chromosome; the correct detection rate of four-dimensional color Doppler ultrasound was 88. 89 %.Among 311 pregnant women,24 cases had fetal malformation,the incidence rate being 7. 72 %(24/311) while the correct detection rate of NT ultrasound combined with four-dimensional color Doppler ultrasound was 91. 67 %(22/24). Conclusion: Fetal NT ultrasound in early pregnancy can reduce the risk of fetal malformation. In the second trimester,four-dimensional color Doppler ultrasound can further clarify the development of the fetus and avoid fetal malformations.
