保留睾丸动脉的术式治疗精索静脉曲张

摘　　要：目的探讨放大镜辅助下精索静脉结扎术与腹膜后精索内静脉高位结扎术的疗效。方法纳入患者81例,随机分为两组,观察组41例行放大镜辅助下精索静脉结扎术,对照组40例行腹膜后精索内静脉高位结扎术,记录两组患者的手术时间、术后住院时间,术后不同时期的精液常规(3、6、9、12个月),观察术后1 a内患者阴囊水肿、附睾炎及睾丸鞘膜积液的发生率,观察术后复发率。结果两组患者手术时间对比,对照组时间短,差异有统计学意义(P<0.05);两组患者术后住院时间,差异无统计学意义(P>0.05);两组患者术后不同时期精子质量均有提高,差异有统计学意义(P<0.05);观察组术后6、12个月精子质量较对照组提高明显,差异有统计学意义(P<0.05);观察组患者术后睾丸鞘膜积液、阴囊水肿及附睾炎的发生率差异无统计学意义(P>0.05),观察组复发率明显小于对照组(P<0.05)。结论放大镜术式较腹膜后术式疗效更好、复发率低,临床效果更优。Objective To study the clinical effect of loupe magnification varicocelectomy and retroperitoneal varicocelectomy to varicocele with abnormal sperm quality. Methods 81 cases were divided into observation group and control group randomly. The experimental group（ 41 patients） wastreated withloupe magnification varicocelectomy. The control group（ 40 patients） was treated with retroperitoneal varicocelectomy. The operation time,postoperative hospital stay,semen analysis of different time after operation（ 3,6,12 moths） were compared between the two groups. Edema of scrotum,epididymitis and hydrocele incidence,postoperative recurrence rate within 1 year after operation were observed. Results The operation time in observation group was longer than that of the control group obviously（ P〈0. 05）. The sperm quality of the two groups was obviously improved at different stages after operation（ P〈0. 05）. There was no statistical difference in sperm quality between the two groups in 3 months after the operation（ P〈0. 05）. The sperm quality in the study group remarkably increased compared with the control group in 6,12 months after the operation（ P〈0. 05）. The postoperative hydrocele,scrotal edema and epididymitis incidence rate were no statistical difference between two groups（ P〉0. 05）. The recurrence rate of observation group was significantly less than that of the control group（ P〈0. 05）. Conclusion The loupe magnification varicocelectomy has better curative effect,less complication,lower recurrence rate and better clinical effect than retroperitoneal high ligation varicocelectomy.
