摘  要:目的评估不屏气扫描模式在老年人肺CT检查中的应用价值。方法选取年龄大于60岁,在我院行肺CT扫描的患者120例,将受检者分为实验组和对照组,每组60人,实验组采用Siemens Definition Force进行扫描,使用CARE k V和CARE Dose4D功能,并且使用不屏气扫描模式。对照组采用Siemens Sensation 16进行螺旋CT扫描,参数设定为100 k V,使用CARE Dose4D功能控制曝光量。2组图像分别取胸锁关节、气管隆突、右下肺静脉三个层面的5 mm轴位图像,在两侧均匀的肺实质内分别设定2个感兴趣区(ROI),记录各组平均CT值、噪声(SD),根据公式计算信噪比(SNR)与对比噪声比(CNR);由2名高年资放射科医师采用双盲法对图像进行评价,并将结果进行比较;记录比较2组的剂量长度乘积(DLP)及有效剂量(ED)。结果实验组和对照组在胸锁关节、气管隆突、右下肺静脉三个层面的肺实质CT值均值分别为(-827.81±36.31),(-834.32±24.88),2组图像的CNR分别为(54.74±10.41),(54.38±5.08),2组数据差异无统计学意义,P>0.05。其扫描时间分别为(0.49±0.04)s,(15.55±1.36)s,DLP为(127.17±49.78),(178.67±44.40),2组数据差异有统计学意义,P<0.001。观察者间有较好的一致性Kappa为0.782。结论不屏气扫描模式在老年人肺CT检查中具有很好的表现,特别是扫描时间缩短在1 s以内,大幅度的减少运动伪影,提高图像质量,对于无意识或不能自主屏气的患者更是提供了解决方案,能满足临床诊断,具有很高的临床应用价值,值得广泛推广。Objective To evaluate the performance of non-breath-hold scanning mode in lung CT examination of the elderly. Methods 120 elderly for lung CT examination, aged over 60, were selected and divided into 2 groups: experiment group and control group, 60 in each; Siemens Definition Force was employed in the examination of the elderly in experimental group, with the parameters of CAREk V and CARE Dose4 D and non-breath-hold scanning mode was adopted while Siemens sensation 16 was employed to perform spiral CT scanning in the examination of the elderly in control group, with the parameters of 100 k Vand CARE Dose4 D; 3 slices of 5-mm axial images of the sternoclavicular joint, the tracheal carina and the right inferior pulmonary vein were taken; 2 regions of interest(ROI) were designated on both sides of lung parenchyma and the CT averages,noise(± standard deviation), signal-to-noise ratio(SNR), and contrast-to-noise ratio(CNR) were recorded; the images were evaluated by 2 senior radiologists via double-blind method and the results were compared, including the dose length product(DLP) and the effective dose(ED) in the 2 groups. Results The CT average of lung parenchyma in experimental group was(-827.81±36.31) while that in control group was(-834.32±24.88), the difference between the 2 groups was of no statistical significance(P 0.05); no statistical differences were found in CNR between the 2 groups( P0.05); the scanning time and DLP were(0.49 ± 0.04) s and(127.17 ± 49.78) in experiment group while those were(15.55 ±1.36) s and(178.67 ±44.40) in control group, the differences were of statistical significance( P0.001); a good consistency was found between the 2 senior radiologists with Kappa value of 0.782. Conclusions The non-breath-hold scanning mode has a good performance in lung CT examination of the elderly, especially when the scanning time is shortened within 1 second, greatly reducing the motion artifact and improving the image quality; it provid
