系统性红斑狼疮合并动脉粥样硬化与外周血维生素D水平的相关性分析

摘　　要：目的检测系统性红斑狼疮(systemic lupus erythematosus,SLE)患者颈动脉内中膜厚度(intima-media thickness,IMT)及外周血中的25-羟基维生素D(25-Hydroxyvitamin D,25-OH-D)水平,探讨维生素D与SLE合并动脉粥样硬化(atherosclerosis,AS)的相关性。方法超声检查60例SLE患者合并AS(SLE组)及30例健康体检者(健康对照组)的颈动脉,并用电化学发光方法检测患者外周血25-OH-D水平,分析SLE合并AS患者与外周血维生素D的相关性。结果 1)SLE组合并AS发病率高于健康对照组(P<0.05);2)SLE组(包括IMT阳性组及IMT阴性组)25-OH-D水平低于对照组(P<0.05),IMT阳性组25-OH-D水平低于IMT阴性组(P<0.05);3)SLE组IMT与血清25-OH-D水平呈负相关(r=-0.345,P<0.01)。结论SLE合并AS的患者,其外周血25-OH-D水平比未合并AS的SLE患者及健康人群要低,可能与SLE合并AS形成有关。Objective To detect the carotid intima-media thickness（IMT）and serum 25-hydroxyvitamin D（25-OH-D）levels in patients with systemic lupus erythematosus（SLE）,and to investigate the relationship between 25-OH-D levels and SLE complicated by artherosclerosis（AS）.Methods Carotid IMT was measured by ultrasound and serum 25-OH-D levels were detected using electrochemiluminescence method in 60 SLE patients and 30 healthy controls.The correlation between SLE complicated by AS and vitamin D levels was analyzed.Results Compared with control group,the incidence of AS increased and serum 25-OH-D levels decreased in SLE group（P〈0.05）.Furthermore,serum 25-OH-D levels in IMT-positive patients were lower than those in IMT-negative patients（P〈0.05）.The IMT was negatively correlated with 25-OH-D levels in patients with SLE（r=-0.345,P〈0.01）.Conclusion Peripheral blood 25-OH-D levels in SLE patients with AS are lower than those in SLE patients with without AS and healthy persons,suggesting that 25-OH-D may play an important role in the occurrence of atherosclerosis in patients with SLE.
