摘 要:目的探讨新诊断2型糖尿病患者血清A-FABP与YKL-40水平的变化及其与胰岛素抵抗的关系及二者之间的相关性。方法选取佳木斯大学附属第一医院内分泌科就诊的新诊断2型糖尿病患者60例(T2DM组),另选体检健康者60名作为健康对照组(NC组)。采用ELISA检测血清A-FABP和YKL-40水平。结果 T2DM组血清A-FABP和YKL-40水平均高于NC组,差异均有统计学意义(P<0.01);A-FABP与体重、BMI、腰围、臀围、WHR、FPG、Hb A1c、FIns、TG、HOMA-IR呈正相关,与QUICKI、HDL呈负相关(均P<0.05);YKL-40与FPG、Hb A1c、FIns、HOMA-IR呈正相关,与QUICKI、HDL呈负相关(均P<0.05);A-FABP与YKL-40呈正相关(P<0.05);多元逐步回归分析结果显示,HOMA-IR和BMI是血清A-FABP水平的独立影响因素,HOMA-IR和FPG是血清YKL-40水平的独立相关因素。结论 A-FABP可能参与调节YKL-40的生成,二者在胰岛素抵抗的发生发展中可能拥有共同的相关信号通路。Objective To investigate the relationship and correlation between serum A-FABP and YKL-40 levels and insulin resistance in newly diagnosed type 2 diabetic patients. Methods 60 newly diagnosed patients with type 2 diabetes mellitus treated in our hospital were selected as the experiment group(T2DM group) and 60 healthy subjects were chosen as the control group(NC group).Serum A-FABP and YKL-40 levels were detected by ELISA. Results The levels of serum A-FABP and YKL-40 in T2DM group were higher than those in NC group,and the difference was statistically significant(P〈0. 01). A-FABP was positively correlated with body weight,BMI,waist circumference,hip circumference,WHR,FPG,Hb A1 c,FIns,TG,HOMA-IR and negatively correlated with QUICKI and HDL(both P〈0. 05). YKL-40 was positively correlated with FPG,Hb A1 c,FIns and HOMA-IR,and negatively correlated with QUICKI and HDL(both P〈0. 05). A-FABP was positively correlated with YKL-40(P〈0. 05). Multiple linear stepwise regression analysis showed that HOMA-IR and BMI were independent influencing factors on serum A-FABP levels,and HOMA-IR and FPG independently affect serum YKL-40 levels. Conclusion A-FABP may be involved in regulating the formation of ykl-40,and they may have a common signaling pathway in the occurrence and development of insulin resistance.
