Duckett术式与改良Snodgrass尿道成形术在小儿尿道下裂治疗中的疗效及并发症对比综合分析

摘　　要：目的研究Duckett术式与改良Snodgrass尿道成形术在小儿尿道下裂治疗中的疗效及并发症发生情况。方法本次研究选取的研究对象为2015年10月~2016年10月期间在本院进行治疗的尿道下裂患儿,将70例患儿计算机随机分为两组,各35例。将实施Duckett术式治疗的患儿纳入对照组,将采用改良Snodgrass术治疗的患儿设为观察组。将两组尿道下裂患儿的手术成功率、手术相关指标、并发症发生率进行比对。结果观察组尿道下裂患儿的手术时间、下床时间、出血量、尿道狭窄发生率比对照组更优(P<0.05);两组之间对比手术成功率、住院时间以及尿瘘、阴茎扭转发生率无统计学意义。结论 Duckett术式、改良Snodgrass术两种尿道成形术应用在小儿尿道下裂治疗中均有较好的效果,但改良Snodgrass术的手术相关指标更优,在临床治疗中应根据患儿的尿道板发育情况、阴茎下弯情况等合理选择术式。Objective Duckett and improvement Snodgrass the curative effect and complications of urethroplasty in the treatment of hypospadias in children. Methods The subjects of this study were children with hypospadias who were treated in our hospital from October 2015 to October2016. 70 patients were randomly divided into two groups, 35 cases in each group. Children who carried out Duckett therapy were included in the control group, and the children treated with improved Snodgrass were set up as the observation group. Two groups of children with hypospadias, surgical success rate, surgical related index and incidence of complications were compared. Results The observation group of hypospadias operation time,bed time, bleeding volume, the incidence of urethral stricture is better than the control group（P〈0.05）; the two group comparison between the surgical success rate, hospitalization time and incidence of urinary fistula, torsion of penis was not statistically significant. Conclusion Two kinds of urethroplasty with Duckett operation and modified Snodgrass have good effect in the treatment of hypospadias in children, but the operative index of modified Snodgrass is better. According to the development of the urethra plate and the condition of the penis, the surgical treatment should be chosen in the clinical treatment.
