手法复位结合手术治疗Sanders Ⅲ Ⅳ型跟骨骨折疗效分析

摘　　要：目的评价应用术前手法复位结合切开复位接骨板内固定手术治疗SandersⅢ、Ⅳ型跟骨骨折的疗效。方法对我院2009年1月—2014年8月收治的68例80足SandersⅢ、Ⅳ型跟骨骨折患者,入院后即给予手法复位,待消肿后再行切开复位接骨板内固定。随访评价患者的术前消肿时间、骨折愈合时间、关节功能、伤口情况。结果 68例患者均获随访,时间6个月~18个月,平均10.1个月。术前消肿时间为3 d^5 d,术后骨折全部愈合,愈合时间10周~12周。按照美国足踝协会后足评分(AOFAS)进行功能评定,优38例,良20例,可10例,优良率为85%。本组患者中有3例于术后1周出现切口转弯处伤口渗出,经换药后于术后3周愈合。结论术前手法复位结合切开复位接骨板内固定治疗SanderⅢ、Ⅳ型跟骨骨折可以获得良好疗效。Objective To evaluate the clinical effects of manual reduction combined with open reduction of anatomical plate fixation in treating calcaneal fractures. Methods From January 2009 to August 2014,Sixty-eight patients（80 feet）with calcaneal fractures（Sanders Ⅲ、Ⅳ）were reviewed. As soon as admission,The patients were treated with manual reduction and with anatomical plate fixation after the swellings reduced. Preoperative swelling time、the average period of fracture healing time,joint function and complications were evaluated. Results 68 patients were followed up,and the duration ranged from 6 to 18 months,with a mean of10.1 months. The pain was markedly relieved at the next day after reduction,and the swelling was relieved in 3 to 5 days. According to AOFAS foot function score system,38 patients got an excellent result,20 good,10 fair. The total excellent and good results were 85%. There were 3 patients in this group at 1 weeks after surgery and wound leakage at the wound turning,and 3 weeks later healing after dressing change. Conclusion Preoperative manual reduction combined with open reduction and internal fixation calcaneal fractures（Sanders Ⅲ,Ⅳ）can achieve good curative effect.
