中药熏蒸联合功能锻炼促进自体动静脉內瘘成熟的临床研究

摘　　要：目的观察中药熏蒸联合功能锻炼促进自体动静脉内瘘成熟的效果。方法将60例首次内瘘术后患者按随机数编秩次分为观察组和对照组各30例,对照组实行常规治疗及功能锻炼,观察组在对照组基础上增加中药熏蒸治疗15d。比较两组患者术后2周、4周、6周头静脉内径及头静脉血流量情况,术后4周、6周内瘘生理成熟率,观察患者内瘘使用1年内并发症发生情况及内瘘通畅度。结果观察组术后4周、6周头静脉内径增宽、血流量增加优于同期对照组,术后6周内瘘生理成熟率高于对照组,内瘘使用1年内并发症发生率低于对照组,内瘘通畅率高于对照组,除内瘘血栓发生率外,比较差异均有统计学意义。结论中药熏蒸联合功能锻炼可促进内瘘成熟。Objective To observe the effect of herbal fumigation combined with functional exercise on maturity of native arteriovenous fistula.Method Divide 60 patients after first fistula operation into observation group（n=30）and control group（n=30）randomly.Patients in control group receive routine treatment and functional exercise.Patients in observation group receive herbal fumigation for 15 dbased on intervention of control group.Compare internal diameter and blood flow of cephalic vein 2,4,6 weeks after operation and physiological maturation rate of fistula 4 and 6 weeks after operation.Observe the complication of fistula in one year and patency rate of fistula.Result Observation group is better on internal diameter and blood flow increasing of cephalic vein than control group 4 and 6 weeks after operation.Physiological maturation rate of fistula 6 weeks after operation is higher in observation group than in control group.The incidence on complication of fistula in one year is lower in observation group.The patency rate of fistula is higher in observation group.The difference is significant between two groups except thrombus rate of fistula.Conclusion Herbal fumigation combined with functional exercise can promote maturity of native arteriovenous fistula
