咽鼓管球囊扩张+鼓膜切开置管治疗难治性分泌性中耳炎的临床效果

摘　　要：目的研究并探讨咽鼓管球囊扩张+鼓膜切开置管联合治疗难治性分泌性中耳炎的临床效果。方法选取2015年1月~2017年6月我院收治的60例难治性分泌性中耳炎患者作为研究对象,采用随机数字表法分为两组,每组各30例。对照组采用单纯鼓膜切开置管治疗,观察组采用鼓膜切开置管联合咽鼓管球囊扩张治疗,术后随访6个月,比较两组的临床疗效、纯音测听值、咽鼓管压力、疼痛评分。结果观察组总有效率为93.33%,对照组为73.33%,两组比较差异有统计学意义(P<0.05)。治疗后,观察组的纯音测听值高于对照组(P<0.05),其咽鼓管压力、疼痛评分均低于对照组,差异有统计学意义(P<0.05)。结论鼓膜切开置管联合咽鼓管球囊扩张用于难治性分泌性中耳炎患者中,可有效改善患者听力,减轻其咽鼓管压力和疼痛,具有显著疗效。Objective To study and discuss the clinical effect of eustachian tube balloon dilatation combined with tympanic membrane incision catheter in the treatment of refractory otitis media with effusion.Methods From January2015 to June 2017,60 cases of patients with refractory secretory otitis media treated in our hospital were selected and divided into two groups by random number table method,30 cases in each group.The control group was treated by simple tympanic membrane incision catheter,the observation group was treated by tympanic membrane incision catheter combined with eustachian tube balloon dilatation.The patients were followed up for 6 months,the clinical effects,pure audiometry,eustachian tube pressure and pain score were compared between the two groups.Results The total effective rate in the observation group was 93.33%,the control group was 73.33%,there was statistically significant between the two groups（P〈0.05）.After treatment,the pure tone audiometry in the observation group was higher than that in the control group（P〈0.05）,eustachian tube pressure and pain scores in the observation group were lower than that in the control group,the difference was statistically significant（P〈0.05）.Conclusion Tympanic membrane incision catheter combined with eustachian tube balloon dilatation can be used to treat patients with refractory otitis media with effusion,which can effectively improve hearing and relieve pressure and pain of eustachian tube,it has significant effect.
