七氟醚吸入麻醉对小儿疝气手术患者血流动力学及麻醉质量的影响

摘　　要：目的:探讨七氟醚吸入麻醉对小儿疝气手术患者血流动力学及麻醉质量的影响。方法:选取2016年1月至2017年3月收治的接受手术治疗的疝气患儿80例为研究对象,随机分为观察组(n=40)和对照组(n=40),对照组采用丙泊酚复合舒芬太尼静脉麻醉,观察组采用经喉罩七氟醚吸入麻醉。比较两组患儿麻醉诱导前(T_0)、插入喉罩/气管导管前(T_1)、插入喉罩/气管导管后即刻(T_2)、插入喉罩/气管导管后10 min(T_3)、拔管即刻(T_4)的SBP、DBP和HR水平。比较两组患儿的拔管时间、复苏时间和相关呼吸道并发症发生情况。结果:对照组T_2与T_4时机SBP、DBP和HR明显高于T_0时机,T_2与T_4时机对照组SBP、DBP和HR明显高于观察组,差异均有统计学意义(P<0.05);观察组拔管时间、复苏时间和呼吸道并发症发生率均小于对照组,差异均有统计学意义(P<0.05)。结论:与丙泊酚静脉麻醉相比,喉罩吸入七氟醚用于小儿疝气手术麻醉效果显著且安全性较高,值得推广应用。Objective： To investigate the effect of sevoflurane inhalation anesthesia on hemodynamics and anesthesia quality in children undergoing hernia surgery. Methods： Eighty patients with hernia admitted for surgical treatment were from January 2016 to March 2017,randomly divided into observation group and control group with 40 cases in each. The control group was treated with intravenous anesthesia with propofol,while observation group was treated with laryngeal mask anesthesia with sevoflurane. SBP,DBP and HR levels of two groups before anesthesia induction（ T_0）,before intubation（ T_1）,intubation（ T_2）,10 min after intubation（ T_3）,immediately after extubation（ T_4）. Extubation time,recovery time,and complications of respiratory tract of the two groups were compared. Results： SBP,DBP and HR for T_2 and T_4 of control group were higher than those for T_0. In T_2 and T_4,SBP,DBP and HR of control group were significantly higher than those of observational group（ P〈0.05）. Conclusion： Compared with intravenous anesthesia using propofol,laryngeal mask anesthesia using sevoflurane was more effective and safer,and is worthy of clinical application in children undergoing hernia surgery.
