摘  要:目的观察脑脊液降钙素原在颅内细菌性感染与无菌性脑膜炎患者中的鉴别效果及临床价值。方法取颅内细菌性感染(n=43)与无菌性脑膜炎患者(n=17),设为观察组,取同期入院健康体检者60例,设为对照组。采用双抗夹心免疫发光法测定血清降钙素原水平,采用全自动速率散射比浊法测定C反应蛋白;采用全自动生化分析仪测定患者WBC水平,分析脑脊液降钙素原在颅内细菌性感染与无菌性脑膜炎中的鉴别效果。结果两组体温差异无统计学意义(P>0.05);观察组血清降钙素原、WBC计数及C反应蛋白水平,显著高于对照组(P<0.05);颅内细菌性感染与无菌性脑膜炎患者体温比较差异无统计学意义(P>0.05);颅内细菌性感染患者脑脊液降钙素原、WBC计数、C反应蛋白,高于无菌性脑膜炎(P<0.05)。降钙素原、WBC计数和C反应蛋白诊断敏感性、特异性指标比较差异无统计学意义(P>0.05);降钙素原、WBC计数和C反应蛋白联合诊断检测敏感性和特异性指标,高于单一指标(P<0.05)。结论脑脊液降钙素原在颅内细菌性感染与无菌性脑膜炎患者中鉴别效果理想,能指导临床治疗,值得临床应用。Objective To observe the cerebrospinal fluid procalcitonin in intracranial bacterial infection and aseptic meningitis in the differential diagnosis and clinical value. Methods Sixty patients with bacterial infection(n = 43) and asymptomatic meningitis(n=17) were enrolled in the study. 60 healthy subjects were enrolled in this study. The level of serum procalcitonin was measured by double-antibody sandwich immuno-luminescence method. C-reactive protein was measured by automatic rate turbidimetric turbidimetric method. WBC level was measured by automatic biochemical analyzer. The content of procalcitonin in cerebrospinal fluid Infection and aseptic meningitis. Results There was no significant difference in body temperature between the two groups(P 0.05). Serum procalcitonin, WBC count and C-reactive protein in the observation group were significantly higher than those in the control group(P 0.05). The cerebrospinal fluid procalcitonin, WBC count and C-reactive protein in cerebrospinal fluid of patients with intracranial bacterial infection were higher than those of aseptic meningitis(P 0.05).(P 0.05). The sensitivity and specificity of the combination of calcitonin, WBC count and C-reactive protein were significantly higher than those of the control group(P 0.05), but there was no significant difference in the sensitivity and specificity, Which was higher than the single index(P 0.05). Conclusion Cerebrospinal fluid procalcitonin is an ideal agent in the diagnosis of intracranial bacterial infection and aseptic meningitis. It can be used as a guide for clinical treatment.
