摘  要:目的比较盐酸戊乙奎醚(PHC)、右美托咪定(DEX)对内毒素致大鼠急性肺损伤(ALI)的肺保护作用。方法 40只清洁级健康成年雄性SD大鼠,随机数字表法分为4组,各10只。0.9%氯化钠注射液对照组(NS组)、内毒素致大鼠肺损伤模型组(ALI组)、DEX治疗组(L+D组)、PHC治疗组(L+P组)。ALI组、L+D组、L+P组均采用内毒素(LPS)6 mg/kg经股静脉注射建立大鼠ALI模型。L+D组在LPS注射0.5 h后2min内股静脉注射DEX50μg/kg,随后以5μg·kg^(-1)·h^(-1)泵入,直到实验结束;L+P组在LPS注射0.5h后2 min内股静脉注射PHC 0.5 mg/kg,随后以50μg·kg^(-1)·h^(-1)泵入,直到实验结束;对照组注入等量0.9%氯化钠注射液。以6h为实验终点,采取颈动脉血2 ml、制备肺组织标本,随后放血致死。用酶联免疫吸附(ELISA)法测定血浆肿瘤坏死因子(TNF-α)、白细胞介素(IL-1β)及白细胞介素-6(IL-6)水平;取右中肺叶,称重后计算肺湿/干重比(W/D)值;取右肺上叶,光镜下观察病理结果并进行弥漫性肺泡损伤标准(DAD)评分;左肺支气管灌洗,采集肺泡灌洗液(BALF),测定TNF-α、IL-6、总蛋白(TP)水平。结果 LPS静脉注射后肺组织发生损伤性改变。3个实验组血浆TNF-α、IL-1β及IL-6均高于对照组(均P<0.05);L+D组、L+P组血浆TNF-α、IL-11β、IL-6均低于ALI组(均P<0.05);L+D组与L+P组差异无统计学意义(P>0.05)。3个实验组W/D均高于对照组(均P<0.05);L+D组、L+P组W/D均低于ALI组(均P<0.05);L+D组与L+P组差异无统计学(P>0.05)。3个实验组病理损伤明显,DAD评分均高于对照组(均P<0.05);L+D组与L+P组的病理损伤较轻、DAD评分均低于ALI组(均P<0.05);L+D组与L+P组差异无统计学意义(P>0.05)。3个实验组BALF中的TNF-α、IL-6、TP均高于对照组(均P<0.05);L+D组、L+P组BALF中的TNF-α、IL-6及TP均低于ALI组(均P<0.05);L+D组与L+P组差异无统计学意义(P>0.05)。结论 PHC或DEX治疗均对LPS致大鼠ALI有保护性作用。Objective To compare the protective effects of penehychdine hydrochloride injection and dexmedeto- midine in rats with acute lung injury(ALl) induced by endotoxine. Methods Forty adult male SD rats were ran- domly divided into four groups, with 10 rats in each group. These four groups included 0.9%NS group, group ALl (acute lung injury caused by endotoxine), dexmedetomidine (DEX) treatment group (L+D group) and penehychdine hydrochloride treatment group (L+P group). ALI rats models were established with the way of administering intra- venous injections of LPS. In L+D group, 0.5 hours after the femoral intravenous injection of LPS 6 mg/kg, 50 μg/kg DEX thenwas injected within two mins, followed by amaintenance infusion of 5 μg . kg- 1 , h- 1 dexrnedetomidine. Analogously, in L+P group, 0.5 hours aRer the injection of LPS, 0.5 mg/kg penehychdine hydrochloride was injected, followed with a maintenance infusion of 50 μg . kg - 1 . h- 1 penehychdine hydrochloride until the end. By contrast, In group NS, the same amount dose of 0.9% sodium chloride solution was intravenously injected in order to serve as a control group. Six hours after all injections, the experiment ended. Their carotid blood were collected (2ml) and lung tissue specimens were prepared, and then all rats were sentenced to death. Samples of collected carotid blood were taken to measure the levels of TNF-α, IL-1 β and IL-6. The superior lobe of right lung was examined under light microscope. Histologic findings from superior lobe of right lung were set as references of lung tissue injury scores(DAD scores). The middle lobe of right lung was used to calculate the lung tissue wet weight/dry weight (W/D) ratio. The left bmnchoalveolar of dead rats was performed by a lavage. Then the bronchoalveolar lavage fluid (BALF) was collected to measure the levels ofTP, TNF-α and IL-6. Results Injuries on lung tissue hap- pened after LPS was intravenously injected.
