颈前路手术后出现早期并发症的相关因素分析

摘　　要：目的探讨颈前路手术后出现早期并发症的相关因素,为临床提供参考。方法回顾性分析我科2011-05-2015-05行颈前路手术治疗的173例患者,其中18例发生早期并发症,包括颈前血肿3例,吞咽困难8例,脑脊液漏4例,内固定移位2例,神经根损伤1例,总发生率为10.4%。现将此18例作为有并发症组,未出现术后早期并发症的另155例作为无并发症组,进行相关因素的统计学分析。结果单因素分析显示,在性别、平均年龄、BMI指数和高血压、糖尿病、吸烟、酗酒、疾病类型、手术节段、手术方式、手术时间和出血量等指标的单因素分析中,平均年龄、手术节段在两组之间有显著的统计学意义(P<0.05);多因素Logistic回归分析也证实,平均年龄、手术节段是两项独立的危险因素。结论年龄和手术节段是影响颈前路手术后早期并发症的独立危险因素,患者年龄越高、手术节段为C_(3-4)节段者,术后发生早期并发症的可能性越高,应引起临床重视。Objective To investigate the related factors of early complications after anterior cervical surgery, and to provide reference for clinical practice. Methods From May 2011 to May2015, the clinical data of 173 patients who underwent anterior cervical spine surgery in our department were retrospectively analyzed. Of 173 patients, 18 cases with early complications, including3 cases of anterior cervical hematoma, 8 cases of dysphagia, 4 cases of cerebrospinal fluid leakage,internal fixation displacement in 2 cases, 1 case of nerve root injury, the total incidence rate was10.4%. These 18 cases were selected as the complication group, the other 155 cases without early postoperative complications were selected as the no complications group. The related factors were statistically analyzed. Results Univariate analysis showed that, in the analysis of average age, gender,hypertension, diabetes, BMI index and smoking, alcohol abuse, disease type, surgical segment,operation method, operation time and the amount of bleeding in operation, the average age and surgical segment had statistically significant differences between the two groups（P〈 0.05）. Logistic regression analysis also confirmed that the average age and surgical segment were two independent risk factors.Conclusion Age and surgical segment are independent risk factors of early complications of anterior cervical surgery after surgery. Patients with higher age, the segment for the C_（3-4） segment, the possibility of early complications after surgery is high, which should be paid more attention.
