摘 要:目的:应用功能磁共振成像技术(fMRI)观察针刺右侧足太阳膀胱经原络穴(京骨、飞扬穴)引起的脑效应,分析其特征及临床意义,并探讨针刺京骨、飞扬穴脑效应机制。方法:选取符合标准的14名健康受试者,针刺右侧京骨、飞扬穴(由同一操作者将针顺次刺入两穴后,同时捻针),获取Bold(blood oxygen level dependent)脑激活区信号的数据,由3.0 T TX MRI扫描仪采集,并应用SPM12(statistical parametric mapping)软件包处理和分析。结果:针刺京骨、飞扬穴的正激活脑区包括:双侧梭状回,右侧小脑后叶、颞上回(BA22)、额上回(BA6),左侧小脑前叶、颞中回;未见明显负激活。结论:针刺京骨、飞扬穴激活脑功能区与膀胱经主一身之筋功能相关,且对听皮层联合中枢及次级运动皮层的血液循环影响较明显。Objective: To observe the brain response by using functional magnetic resonance imaging( fMRI)when needling right Jinggu( BL64) and Feiyang( BL58),which are Yuan-source acupoint and Luo-connecting acupoint of Bladder Meridian of Foot-Taiyang; and to analyze the distribution,characteristics and its clinical significance of the signal,thus to explore the signal changes of brain after needling these two points.Methods: 14 healthy subjects who met the criteria were given acupuncture by needling right BL64 and right BL58( after the needle is successively inserted into two points by the same operator,twirling the needle simultaneously) to obtain the data of brain activation signal,i. e. Bold( blood oxygen level dependent). The data was collected by the 3. 0 T TX MRI scanner. SPM12( statistical parametric mapping) software package was applied to process and analyze the data. Results: The positive brain regions by needling BL64 and BL58 included:Bilateral fusiform gyrus,right posterior cerebellum,superior temporal gyrus( BA22),superior frontal gyrus( BA6),left cerebellar anterior lobe and middle temporal gyrus. There was no significant negative activation.Conclusion: Needling BL64 and BL58 to activate brain functional areas is related to the function of Bladder governing the tendon of the whole body. It also has a significant effect on the blood circulation of the combined cortex of the auditory cortex and the secondary motor cortex.
