针灸联合康复干预对运动性构音障碍患者疗效及预后的影响研究

摘　　要：目的:观察针灸联合康复干预对运动性构音障碍患者疗效及预后情况。方法:将120例运动性构音障碍患者随机数字法分为对照组与观察组,每组各60例。对照组给予常规言语康复训练,观察组在对照组基础上增加针灸治疗,检测两组临床治疗前后的声学、言语和功能性语言沟通能力相关指标,并统计两组的总有效率。结果:两组治疗后的最大发音时间均显著长于治疗前(P<0.05),且观察组的时间明显长于对照组(P<0.05)。两组治疗后词语、语句和篇章的正确率均显著高于治疗前(P<0.05),且观察组词语和语句的正确率显著高于对照组(P<0.05)。两组治疗1个月后的功能性语言沟通能力得分均显著高于治疗前(P<0.05),且观察组的各项得分显著高于对照组(P<0.05)。观察组治疗总有效率显著高于对照组,组间比较差异有显著性(P<0.05)。结论:针灸联合康复干预治疗运动性构音障碍疗效确切,可有效改善患者的声学和言语障碍,提高患者功能性语言沟通能力,值得临床上推广应用。Objective： To observe the efficacy and prognosis of acupuncture combined with rehabilitation intervention in the treatment of motor dysarthria. Methods： 120 patients with motor dysarthria were randomly divided into the control group（ n = 60） and the observation group（ n = 60）. The control group was given conventional verbal rehabilitation training; on which basis,the observation group was also given acupuncture treatment. The indexes of speech and communication ability of functional language were detected in the two groups before and after the treatment. The total effective rate was also compared between the two groups. Results： The maximum phonation time was significantly longer in the two groups after the treatment,of which the observation group improved more than the control group（ P〈0. 05）. The correct rate of speech was significantly higher after the treatment in both groups（ P〈0. 05）,it was more significantly correct in the observation group than that in the control group（ P〈0. 05）. The scores of speaking,comprehension,reading,writing and other indexes of functional speech communication were significantly higher after one month of treatment in both groups than those before treatment（ P〈0. 05）,and the score of the observation group was significantly higher than that of the control group（ P〈0. 05）. The total effective rate of the observation group was significantly higher than that of the control group（ P〈0. 05）. Conclusion： Acupuncture combined with rehabilitation intervention is effective for motor dysarthria,which can effectively improve the patients ＇ acoustics and speech disorders and improve the communication ability of functional language. It is worth popularizing and promoting in clinic.
