舌针结合利咽组方穴位贴敷治疗中风后吞咽困难临床研究

摘　　要：目的:探讨舌针结合利咽组方穴位贴敷治疗中风后吞咽困难的疗效及安全性。方法:选取90例中风后吞咽困难患者,随机分为舌针组、穴位贴敷组和联合组3组,均持续治疗4周。比较3组吞咽、饮水等症状评分,临床SWAL-QOL量表、VFSS评分及NIHSS量表,比较脑血流图及疗效与并发症。结果:舌针组改善率71.43%、穴位贴敷组改善率69.05%,均低于联合组90.48%(P<0.05);与舌针组、穴位贴敷组相比,联合组治疗后SWAL-QOL、VFSS评分较高,NIHSS评分较低,BA、LVA、RVA水平较高(P<0.05)。结论:舌针结合利咽组方穴位贴敷治疗中风后吞咽困难的疗效显著,有助于改善吞咽功能,提高生活质量。Objective： To investigate the efficacy and safety of tongue needling combined with acupoint application of Liyan compound formula in the treatment of dysphagia after stroke.Methods： 90 patients with post-stroke dysphagia were randomly divided into the tongue needling group,the acupoint application group,and the combined group（ tongue needling ＋ acupoint application）.The treatment course was 4 weeks.Scores of swallowing and drinking water,scores of SWAL-QOL,VFSS and NIHSS were compared,rheoencephalogram was detected by Doppler,as well as efficacy and complications were evaluated in the three groups.Results： The improvement rate was 71.43% in the tongue needling group; it was 69.05% in the acupoint application group;which were both significantly lower than 90.48% in the combined group（ P〈0.05）.Scores of SWAL-QOL and VFSS were higher and NIHSS score was lower,levels of BA,LVA and RVA were significantly higher in the combined group than those in the tongue needling group and those in the acupoint application group（ P 0.05）.Conclusion： The therapy of tongue needling combined with acupoint application of Liyan compound formula has remarkable effect on post-stoke dysphagia,it can significantly improve swallowing function and improve life quality of the patients.
