摘 要:目的冠状动脉慢血流(CSF)对左心室功能的影响尚不明确,本研究应用二维斑点追踪成像(2D-STI)评价CSF对左心室功能的影响。资料与方法前瞻性分析35例CSF患者(CSF组)的左心室功能,选择同期年龄、性别相匹配的30例患者为对照组,应用2D-STI检测左心室心肌纵向应变、径向应变及圆周应变,以及收缩期、舒张早期、舒张晚期纵向应变率、径向应变率、圆周应变率,并与对照组患者进行比较。结果 (1)左心室舒张功能:CSF组左心室二尖瓣血流舒张早期速度E峰、二尖瓣血流舒张早期速度E峰/二尖瓣舒张晚期速度A峰、舒张早期纵向应变率、径向应变率、圆周应变率较对照组减低,差异有统计学意义(P<0.05)。(2)左心室收缩功能:CSF组左心室心肌纵向应变、径向应变、圆周应变、收缩期纵向应变率、径向应变率、圆周应变率较对照组减低,差异有统计学意义(P<0.05)。结论 CSF患者左心室舒张功能及收缩功能已损伤,应引起临床高度重视,并给予相应干预。Purpose The influence of coronary slow flow(CSF) on left ventricular function is not clear. In this study, two-dimensional speckle tracking imaging(2 D-STI) is used to evaluate the effect of CSF on left ventricular function. Materials and Methods The left ventricular function of 35 patients with CSF was prospectively analyzed, 30 age and gender-matched controls were also enrolled, the longitudinal strain, radial strain, circumferential strain, systolic longitudinal strain rate, systolic radial strain rate, systolic circumferential strain rate, early-diastolic longitudinal strain rate, early-diastolic radial strain rate, early-diastolic circumferential strain rate, late-diastolic longitudinal strain rate, late-diastolic radial strain rate and late-diastolic circumferential strain rate were analyzed and compared with those in the control group. Results(1)Diastolic function of left ventricle: compared with the control group, left ventricular early diastolic mitral inflow velocity, early diastolic mitral inflow velocity/late diastolic mitral inflow velocity, early-diastolic longitudinal strain rate, earlydiastolic radial strain rate, early-diastolic circumferential strain rate in CSF group decreased, and there was statistical significance(P〈0.05).(2)Systolic function of left ventricle: compared with the control group, left ventricular longitudinal strain, radial strain, circumferential strain, systolic longitudinal strain rate, systolic radial strain rate, systolic circumferential strain rate in CSF group decreased, and there was statistical significance(P〈0.05). Conclusion The left ventricular diastolic and systolic function in patients with CSF is damaged, and more attention should be paid to give corresponding clinical intervention.
