烧伤湿润暴露疗法及湿润烧伤膏治疗糖尿病足的效果及对创面组织血管内皮生长因子和表皮细胞生长因子水平的影响

摘　　要：目的观察烧伤湿润暴露疗法及湿润烧伤膏(MEBT/MEBO)治疗糖尿病足的临床疗效及其对创面组织血管内皮生长因子(VEGF)和表皮细胞生长因子(EGF)水平的影响。方法将58例糖尿病足溃疡患者随机分为研究组和对照组,每组29例。研究组用MEBT/MEBO处理创面,对照组用重组牛碱性成纤维细胞生长因子处理创面。比较治疗后两组患者的临床疗效、创面局部症状评分,治疗前及治疗后14 d、28 d的创面面积、溃疡深度、创面VEGF和EGF水平。结果治疗28 d后,研究组患者溃疡面积小于对照组、深度浅于对照组、局部症状体征评分低于对照组(均P<0.05);临床疗效优于对照组(P<0.05)。治疗后第14天、28天,研究组VEGF及EGF水平均高于对照组(均P<0.05)。结论 MEBT/MEBO治疗糖尿病足溃疡可以促进创面愈合,这可能与其上调创面生长因子水平有关。Objective To observe the efficacy of moist exposed burn therapy/moist exposed burn ointment（MEBT/MEBO） for diabetic foot and its effect on vascular endothelial growth factor（VEGF） and epidermal growth factor（EGF） of ulcer wounds. Methods Fifty-eight patients with diabetic foot ulcer were randomly divided into study group and control group,with 29 cases in each group.The study group was treated with MEBT/MEBO,while the control group was treated with recombinant bovine basic fibroblast growth factor.The clinical efficacy and symptom score for local wounds after treatment,as well as the wound areas and the levels of VEGF and EGF of ulcer wounds before treatment and after 14 and 28 days of treatment,were compared between the two groups. Results After 28 days of treatment,the area of ulcer was smaller,the depth of ulcer was less,and the scores of local symptoms and signs were lower in the study group compared to those in the control group（all P 〈0.05）;the clinical efficacy in the study group was superior to that in the control group（ P 〈0.05）. On the 14th and 28th day after treatment,the levels of VEGF and EGF in the study group were higher than those in the control group（all P 〈0.05）. Conclusion MEBT/MEBO can promote wound healing in the treatment of diabetic foot ulcer which might be associated with upregulating the levels of growth factors of ulcer wounds.
