不同靶控浓度舒芬太尼对Narcotrend指数的影响

摘　　要：目的探讨不同效应室靶浓度舒芬太尼对Narcotrend指数的影响。方法选择全麻下行肺癌根治术的患者90例,随机均分为6组,对应的舒芬太尼效应室靶浓度(Ce)分别为:0.2 ng/mL(I组)、0.3 ng/mL(Ⅱ组)、0.4 ng/mL(Ⅲ组)、0.5ng/mL(Ⅳ组)、0.6 ng/mL(Ⅴ组)、0.7 ng/mL(Ⅵ组)。记录舒芬太尼输注前(T0)、达到Ce即刻(T1)、1 min(T2)、3 min(T3)、7 min(T4)的Narcotrend指数(NI)、平均动脉压(MAP)、心率(HR)。结果患者各个时点的MAP、HR差异无统计学意义(P>0.05)。Ⅰ～Ⅳ组T1～T4时点的NI与T0差异无统计学意义(P>0.05);Ⅴ组和Ⅵ组T1～T4时点的NI与T0差异有统计学意义(P<0.05),但两组间差异无统计学意义(P>0.05)。结论舒芬太尼效应室靶控浓度低于0.5 ng/mL时NI无显著降低,高于0.6 ng/mL时能显著降低NI,但靶浓度升高至0.7 ng/mL时NI无进一步下降。Objective To evaluate the influence of different effect concentration of sufentanil to Narcotrend index （ NI）. Methods 90 patients with lung cancer radical surgery under general anesthesia were randomly divided into six groups （n = 15/group） according to the sufentanil target effect compartment site concentration （Ce） 0.2 ng/mL （group I）, 0.3 ng/mL （group II）, 0.4 ng/mL （group III）, 0.5 ng/mL （group IV）, 0.6 ng/mL （group V ）, 0.7 ng/mL （group VI）. Narcotrend index, MAP and HR were recorded before induction （T0）, and 0 min （T1）, 1 min （T2）, 3 min （T3）, 7 min （T4） after reached the Ce of sufentanil. Results No statistical difference was found among each group in MAP or HR at different time points （ P 〉0.05）. Compared with the T0, no significant change were found in NI at T 1 ？T4 in group I ？group IV （P 〉0.05）,the NI was significantly lower in group V and group VI at T1 ？T4 （ P 〈0.05）, while the NI had no statistical difference between group V and group VI （P 〉0.05）. Conclusions When the Ce of sufentanil lower than 0.5 ng/mL, NI dose not decrease significantly. The NI decreases significantly when the Ce of sufentanil higher than 0.6 ng/mL, but the NI shows no further decrease when the Ce of sufentanil up to 0.7 ng/mL.
