摘　　要：目的检测原发免疫性血小板减少症(ITP)患者血清中B淋巴细胞活化因子(BAFF)的表达水平。方法研究53例初诊ITP患者,22例健康对照者。ELISA法检测外周血血清BAFF水平,流式细胞术检测30例ITP患者外周血血小板相关抗体PAIg的表达。结果 ITP患者的血清BAFF水平为(1.93±0.52)μg/L,较对照组的(1.07±0.26)μg/L明显升高(P<0.05)。以PAIg表达率≥5%为阳性阈值,将ITP患者分成阳性与阴性两组,两组的BAFF表达水平无统计学差异(P>0.05)。相关性分析结果提示,PAIg阳性组的BAFF水平与PAIg表达呈正相关(r=0.76,P<0.01);PAIg阴性组的BAFF水平与抗体表达无相关(r=0.26,P>0.05)。结论 ITP患者血清中BAFF表达量增加,且与PAIg有良好的相关性。BAFF表达水平增强可能参与了ITP的发病,BAFF可作为诊断ITP的良好指标。Objective To detect the expression of serum B cell activating factor belonging to the TNF family （BAFF） in patients with primary immune thromboeytopenia （ITP）. Methods＇ 53 cases of ITP patients before being treated and 22 normal individuals as controls were studied. The serum levels of BAFF were tested by ELISA. The expression of PAIg in 30 patients with ITP was assayed by flow- eytometry. Results The serum level of BAFF in ITP patients was （1.93 ±0.52）μg/L, signifiantly higher than （1.07 ±0.26）μgg/L of control group （P 〈0.05）. With PAIg expression rate ≥ 5% as a positive threshold, ITP patients were divided into the positive group and negative group, and no signifi- cant difference was found in the BAFF level between two groups （P〉0.05）. The results of association analysis indicated that BAFF level was positively correlated with PAIg level in the PAIg positive group （r 0.76, P〈0.01）, and had no correlation with PAIg levels in the PAIg neg- ative group （r 0.26, P〉0.05）. Conclusions The serum level of BAFF increases and closely relates to the expression of PAIg in the patients with ITP. The elevated level of BAFF may be involved in the pathogenesis ofITP. BAFF can be a good indicator for diagnosing ITP.
