摘 要:目的探讨血清miR-29b作为生物标志物在2型糖尿病(T2D)和前期糖尿病中的诊断价值。方法应用RT-PCR检测24例T2D患者、22例空腹血糖受损(IFG)或葡萄糖耐量受损(IGT)的前期糖尿病患者以及24例健康对照者的血清中miR-29b的表达水平,并通过多因素logistic回归和受试者工作特征(ROC)分析明确miR-29b潜在的诊断价值。结果与健康对照组(1.03±0.38)相比,T2D(0.49±0.24)和IFG/IGT(0.50±0.33)患者的血清miR-29b表达水平显著降低(P<0.05)。多因素logistic回归分析证实低表达miR-29b与T2D(OR:0.51;95%CI:0.16~0.73;P<0.05)和IFG/IGT(OR:0.56;95%CI:0.23~0.79;P<0.05)明显相关。这种关联在考虑年龄、BMI指数和高血压以及其他生化指标后仍然差异有统计学意义。此外,ROC分析显示,血清miR-29b能清楚区分健康对照和T2D(AUC 0.864;95%CI 0.759~0.969;P<0.001)、IFG/IGT(AUC0.846;95%CI 0.737~0.954;P<0.001)患者。结论 T2D和IFG/IGT患者血清miR-29b表达水平显著降低,可以作为T2D和IFG/IGT潜在的诊断指标。Objective To investigate the diagnostic value of serum miR-29 b as a biomarker in type 2 diabetes( T2 D) and pre-diabetes.Methods The expression levels of serum miR-29 b of 24 patients with T2 D,22 patients with impaired fasting glucose( IFG) or impaired glucose tolerance( IGT) and 24 healthy controls were measured by RT-PCR. Multivariate logistic regression and receiver operating characteristic( ROC) analysis were conducted to evaluate potential diagnostic value of miR-29 b. Results Serum miR-29 b expression levels were significantly decreased in patients with T2 D and IFG/IGT individuals,compared with healthy control subjects( P〈0. 05). As determined by multivariate logistic regression analysis,lower miR-15 a expression was significantly associated with T2 D( OR: 0. 51; 95% CI: 0. 16-0. 73; P〈0. 05) and IFG/IGT( OR,0. 56; 95% CI,0. 23 ~ 0. 79; P〈0. 05). This association remained statistically significant following adjustment for age,body mass index and hypertension,as well as other biochemical indicators.Furthermore,ROC analysis revealed that blood miR-29 b distinguished patients with T2 D and IFG/IGT individuals from healthy controls( AUC 0. 864; 95% CI 0. 759-0. 969; P〈0. 001 and AUC 0. 846; 95% CI 0. 737-0. 954; P〈0. 001,respectively). Conclusion Serum miR-29 b expression levels are significantly lower in patients with T2 D and IFG/IGT individuals,and miR-29 b in serum may serve as a potential biomarker for T2 D and IFG/IGT diagnosis.
