细节优化护理下的护理干预措施对维持性血液透析患者内瘘维护的影响

摘　　要：目的:探讨细节优化护理下的护理干预措施对维持性血液透析动静脉内瘘患者内瘘维护的影响。方法:将2016年12月-2017年10月在笔者所在医院进行维持性血液透析的280例患者作为研究对象,按照随机数字分组法分为研究组与对照组。给予对照组常规护理,研究组在常规护理基础上给予细节优化护理干预措施。对比分析两组患者感染发生率、血栓发生率、出血发生率等并发症情况;采用SF-36生活质量量表对两组患者生活质量进行评价。结果:研究组患者并发症发生率(1.42%)显著低于对照组(12.85%),差异有统计学意义(P<0.05);研究组患者内瘘闭塞率(2.85%)和动脉瘤样扩张率(2.14%)明显低于对照组(8.57%、7.14%),差异有统计学意义(P<0.05);研究组患者生活质量评分:健康指数(74.52±9.64)分,心理指数(80.32±10.13)分,情感指数(79.32±7.17)分,评分明显高于对照组的(56.38±5.02)、(63.24±7.58)、(60.22±6.98)分,差异有统计学意义(P<0.05)。结论:对维持性血液透析患者开展细节优化护理,可显著降低患者内瘘并发症发生率,提高患者生活质量,护理效果显著。Objective：To explore the effect of nursing intervention measures on the maintenance of internal fistula of maintenance hemodialysis patients.Method：280 cases of hemodialysis patients in our hospital were treated as the subjects from December 2016 to October 2017,the group was divided into the study group and the control group according to the random number grouping method.Both groups were patients with arteriovenous fistula.To give the control group regular care,the research group gave details to optimize nursing intervention on the basis of routine nursing.The incidence of infection,the incidence of thrombosis and the incidence of bleeding were analyzed in the two groups.The quality of life of two groups was evaluated by sf-36 living quality scale.Result：The study group of patients with complication rate（1.42%） was significantly lower than the control group（12.85%）,the difference was statistically significant（P〈0.05）;the study group patients with fistula occlusion rate（2.85%） and aneurysmal dilatation rate（2.14%） was significantly lower than the control group（8.57%,7.14%）,the difference was statistically significant（P〈0.05）.The quality of life of patients on health index score（74.52±9.64）points,psychological index（80.32±10.13）points,emotional index（79.32±7.17）points was significantly higher than those of the control group of（56.38±5.02）,（63.24±7.58）,（60.22±6.98）points,the difference was statistically significant（P〈0.05）.Conclusion：The details of optimized nursing for maintenance hemodialysis patients can significantly reduce the incidence of complications of internal fistula,improve the quality of life of patients,and have significant nursing effect.
