经肛肠梗阻导管对左半结肠癌性梗阻患者生存质量的影响

摘　　要：目的探讨对急性梗阻性左半结肠癌患者术前放置经肛肠梗阻导管,手术前后观察患者生存质量指数的变化,为提高患者的生存质量提供理论依据。方法 38例左半结直肠癌伴急性肠梗阻患者术前放置经肛型肠梗阻导管,予减压灌洗治疗后,行腹腔镜Ⅰ期切除吻合手术,并对患者手术前及术后3个月生存质量的变化进行评估。结果 38例患者均成功放置经肛型肠梗阻导管。术后进食量、体重及血清白蛋白值恢复较快,白细胞(WBC)、CRP、PCT值术后降至正常。术前HRQOL评分量表总分平均为(66.7±15.4)分,术后3个月明显升高至(75.6±24.7)分,患者生存质量较术前明显提高,差异有统计学意义(P<0.05)。结论左半结肠癌患者术前放置经肛肠梗阻导管行I期切除吻合术,术后患者恢复快,生存质量高。Objective To investigate the effects of ileus tube application before operation on survival quality of patients with left hemicolon cancerous obstruction in order to provide theoretical basis for improvement of survival quality of patients with left hemicolon cancerous obstruction. Methods Thirty-eight patients with left hemi-colorectal cancer complicated by acute intestinal obstruction received transanal ileus tube after decompression lavage,who were treated by laparoscopy stage I radical resection and anastomosis,then the quality of life of patients before operation and at 3 months after operation was evaluated by means of HRQOL score. Results The ileus tubes were successfully placed in the 38 patients,and postoperative food-intake amount,body weight and serum albumin levels recovered quickly. Moreover the levels of leukocyte,CRP and PCT were decreased to normal levels after operation. The mean total scores of HRQOL scale before operation were（ 66. 7 ± 15. 4）points,which were significantly increased to（ 75. 6 ± 24. 7） points at 3 months after operation,moreover the survival quality of patients was significantly improved,as compared with that before operation（ P〈0. 05）. Conclusion The application of transanal ileus tube before operation in patients with left hemi-colorectal cancer receiving stage I radical resection and anastomosis has the advantsges of faster recovery after operation and higher life quality of patients.
